方法:这是对PAULA研究中1381例患者的辅助分析。PAULA研究为一项横断面、回顾性、全国范围内的多中心研究。
结果:共有370例患者eGFR<60 mL/min/1.73 m2。CKD每个分期的抗凝控制水平渐进地恶化。多线性回归分析显示,CKD是治疗窗(TTR)的一个独立预测因素。在保存肾功能的患者亚组中,女性、饮食影响INR、联合用药和胺碘酮与较差的TTR相关。SAMe-TT2R2评分对TTR<65%有显着但中度的预测价值(AUC,曲线下面积0.558,P=0.002)。在CKD患者亚组中,SAMe-TT2R2(>2分)显示对TTR没有显着预测能力(AUC 0.528,P=0.354)。男女性别的平均TTR相似(P=0.255),但TTR≥65%的男性受试者的百分比更高(P=0.013)。
结论:慢性肾病与使用VKA的非瓣膜性AF患者较差的抗凝控制相关。SAMe-TT2R2评分不能预测CKD亚组中较差的TTR,尽管在非CKD患者中发现对较差TTR具有中等的预测价值。
