J Rheumatol:针对柏林和SPARCC MRI评分法的半轴扫描平面MRI骶髂关节评分法

在半轴扫描平面上SIJ BME的检测是可行和可重复的。

这项研究旨在阐明用于评估轴向脊柱关节炎患者骶髂关节(SIJ)骨髓水肿(BME)的半轴磁共振成像(MRI)评分方法,并与等效的半冠状评分方法比较其可靠度。

基于半冠状柏林和加拿大脊柱关节炎研究联盟(SPARCC)方法的原理开发了两种半轴SIJ MRI评分方法。还开发了一种全面象限法。对参加随机、双盲、阿达木单抗每两周40 mg与安慰剂对照试验的51例患者,使用半轴和对应的半冠状方法对SIJ基线和12周的MRI进行评分。通过线性回归分析比较结果。通过等级内相关系数(ICC)和最小可检测变化[SDC,绝对值和最高观察评分百分比(SDC-HOS))评估可重复性和灵敏度。

半轴评分方法的阅片者间和阅片者内ICC(基线为0.83-0.88和0.85-0.97;变化为0.33-0.78)为中等至非常高,而半冠状评分方法为高至非常高(基线为0.90-0.92和0.93-0.97;变化0.77-0.89)。柏林和SPARCC方法半轴和半冠状评分之间的关联高(基线:R 2 = 0.93和0.88;变化:R 2分别为0.82和0.87),而全面方法较低(基线:R 2 = 0.79 ;变化:R 2 = 0.54)。半轴和半冠状方法的SDC-HOS分别为9.8-18.6%和5.9-10.7%。

在半轴扫描平面上SIJ BME的检测是可行和可重复的。然而所有这3种半轴方法的可靠性略低,支持在治疗性研究中使用冠状扫描平面的一般做法。
