JAMA Pediatr:婴儿急性颅内出血的新型血清学鉴定模式!

近期一项发表在杂志JAMA Pediatr上的文章检测并验证了一组生物标志物,可以作为多变量模型的基础,使用Ziplex系统识别出婴儿颅内出血。

此项研究使用二元逻辑回归开发了包含3个血清生物标志物(基质金属蛋白酶-9、神经元特异性烯醇化酶和血管细胞粘附分子-1)和1个临床变量(总血红蛋白)的多变量模型。然后对该模型进行前瞻性验证。在Ziplex系统上使用Flow-Thru微阵列技术进行多重生物标志物测量。该模型在1级儿科创伤中心(匹兹堡儿童医院,宾夕法尼亚州,匹兹堡;犹他州盐湖城小学儿童医院和伊利诺伊州芝加哥的Lurie儿童医院)进行了3个儿科急诊部门的测试。在受试婴儿中,一些症状使他们面对虐待性头部创伤的风险增加。此项研究于2006年11月至2014年4月在匹兹堡儿童医院,2010年6月至2013年8月在小学儿童医院进行,2011年1月至2013年8月在Lurie儿童医院进行。

此项研究结果显示:多变量模型“婴幼儿脑损伤评分生物标志物”被应用于599例患者中。受试者平均(SD)年龄为4.7(3.1)个月。52%是男孩,78%是白人,8%是西班牙裔。截点为0.182时,急性颅内出血模型敏感性为89.3%(95%CI,87.7-90.4)和特异性为48.0%(95%CI,47.3-48.9)。阳性和阴性预测值分别为21.3%和95.6%。该模型对于无创性脑异常,孤立性颅骨骨折或慢性颅内出血既不敏感也不特异。

此项研究结果表明:使用上述3种血清生物标志物浓度和血清血红蛋白的多变量模型,用于婴儿脑损伤评分的生物标志物可以鉴定具有急性颅内出血的婴儿。婴儿颅内出血判断的准确性和及时性具有降低发病率和死亡率的可能性。
