肿瘤浸润淋巴细胞(TIL)在乳腺癌中的预后价值已得到公认。然而,针对BRCA相关乳腺癌的特定T细胞亚群的研究仍较少。
免疫标记物表达的分布
在该研究中,研究人员共分析了来自414 名携带BRCA突变的乳腺癌患者的肿瘤组织,通过免疫组化和数字图像分析来检测CD4、CD8和FOXP3免疫标志物的表达水平。根据分析低表达组或高表达组来评估CD4、CD8和FOXP3阳性细胞的分布和临床病理特征。并在患者总生存期和无病生存期的单变量和多变量分析中评估其预后价值。
患者的存活率分析
结果显示,CD4和CD8的表达水平均与疾病的组织学诊断、肿瘤分级以及雌激素和孕激素受体表达状态相关。CD4的表达水平与BRCA基因状态相关。研究人员发现,高比例的肿瘤浸润CD4、CD8或FOXP3阳性细胞与BRCA1和BRCA2相关乳腺癌的较低死亡率显著相关,而CD8阳性细胞与患者的无病生存率相关。根据BRCA基因状态,并未发现免疫标志物预后价值的异质性。
总而言之,该研究结果支持了特定T细胞亚群在BRCA相关乳腺癌中的预后作用,以及靶向免疫系统治疗该疾病的潜力。
