Clin Endocrinol:HIV感染女性存在内分泌紊乱?

随着艾滋病的流行,人们发现HIV与很多复杂的激素、生殖和代谢异常有关。为此,研究人员调查了一组HIV阳性的女性内分泌功能紊乱的情况,明确相关因素,并通过测量白细胞端粒的长度(LTL)检测细胞的衰老是否与内分泌功能紊乱有关。研究纳入了CARMA队列研究中年龄≥19岁、HIV+的女性192例。糖尿病的患病率、糖耐量异常、血脂异常、甲状腺疾病、肾上腺、性腺功能减退、原发性卵巢功能不全(POI)、人口特征

随着艾滋病的流行,人们发现HIV与很多复杂的激素、生殖和代谢异常有关。为此,研究人员调查了一组HIV阳性的女性内分泌功能紊乱的情况,明确相关因素,并通过测量白细胞端粒的长度(LTL)检测细胞的衰老是否与内分泌功能紊乱有关。

研究纳入了CARMA队列研究中年龄≥19岁、HIV+的女性192例。糖尿病的患病率、糖耐量异常、血脂异常、甲状腺疾病、肾上腺、性腺功能减退、原发性卵巢功能不全(POI)、人口特征、HIV和丙型肝炎病毒(HCV)感染状况、基线水平的LTL、联合抗逆转录病毒治疗(CART)和物质暴露等情况均需被收集。统计采用单变量及多变量泊松回归分析、逐步回归分析。

结果显示,内分泌功能异常的患者占58%(血脂异常占43%,糖耐量异常/2型糖尿病占13%,甲状腺功能异常占15%)。多变量分析提示年龄与内分泌异常的数量及类型(糖代谢异常、脂代谢异常、任一异常)相关,BMI的升高与糖代谢紊乱有关。有趣的是,峰值HIV病毒数量≥100000拷贝/ml与任一形式的内分泌异常、全部异常以及甲状腺功能异常都有关。但糖代谢异常与脂代谢异常与饮酒无关。LTL与任何形式的内分泌异常均无关。

更进一步的研究探索HIV暴露时间和暴露程度与内分泌功能异常之间的关系是十分必要的。
