Neurology:抗癫痫药与认知功能障碍无关

近日,神经病学领域权威取杂志Neurology上发表了一篇研究文章,该研究旨在检验单个抗癫痫药(AED)与认知障碍无关的假说,研究人员对住院视频EEG监护单元的患者进行了横断面研究。

研究人员对2009年至2016年期间接受住院专科癫痫治疗计划的患者进行了前瞻性研究。评估包括客观认知功能、生活质量主观认知功能量表以及焦虑和抑郁症状问卷。贝叶斯模型识别认知功能预测因子,贝叶斯模型选择方法调查了单个AED对认知功能的影响。研究人员还进行了常规的频谱分析。

该研究共有331例患者符合入选标准。平均年龄为39.3岁,61.9%的患者为女性。总共有45.0%的患者被开具AED多药处方处方,25.1%的患者为AED单药治疗和29.9%的患者无AED处方。年龄、癫痫发作频率以及伴随性癫痫和精神病性非癫痫性发作的诊断是客观认知功能的预测指标。抑郁、焦虑和癫痫发作频率是主观认知功能的预测指标。除其他临床相关变量外,单个AED与认知功能受损之间并没有独立相关。

由此可见,这项研究发现没有AED与认知功能障碍之间独立相关。客观和主观认知功能障碍的重要决定因素分别包括癫痫发作频率和抑郁。这些结果表明,优化治疗以预防癫痫的方法不太可能会以牺牲认知功能为代价。
