JAHA:性别对阿司匹林血小板反应性的影响

研究人员在血小板对阿司匹林的反应中观察到了激动剂依赖性的性别差异。尽管对环氧合酶-1的抑制作用较强,但每天服用阿司匹林的女性对肾上腺素和ADP的反应随时间推移反而会降低对血小板抑制作用,而男性则不然。

阿司匹林预防心肌梗塞和卒中的疗效和安全性存在性别差异。是否可以通过血小板反应性和阿司匹林反应的根本差异来解释这一点尚未明确。

近日,血管疾病领域权威杂志JAHA上发表了一篇研究文章,健康志愿者(n=378208名女性)和冠心病患者或有冠心病危险因素的患者(n=217112名女性)服用阿司匹林治疗4周。阿司匹林首次给药后3小时和每日阿司匹林治疗后3小时,研究人员使用富含血小板血浆的透光度聚集法测定了血小板对肾上腺素、胶原蛋白和ADP的反应性,并对一部分患者进行了药代动力学和药效学评估,评估了水杨酸酯和环加氧酶-1衍生的前列腺素代谢产物的含量,并对花生四烯酸和离体暴露于阿司匹林后进行了凝集测定。

基线时,女性对ADP和胶原蛋白的反应使血小板聚集增加。用基线血小板离体阿司匹林暴露评估,血小板对阿司匹林的反应没有性别差异。首次口服阿司匹林后3小时,女性对肾上腺素的反应在更大程度上抑制了血小板凝集,而胶原蛋白则在较小程度上抑制了血小板凝集。治疗4周后,尽管水杨酸盐浓度更高且环氧合酶-1抑制作用更强,但女性对肾上腺素和ADP的血小板抑制作用有所减弱。

由此可见,研究人员在血小板对阿司匹林的反应中观察到了激动剂依赖性的性别差异。尽管对环氧合酶-1的抑制作用较强,但每天服用阿司匹林的女性对肾上腺素和ADP的反应随时间推移反而会降低对血小板抑制作用,而男性则不然。
