Clin Infect Dis:使用ISDD装置有效避免血培养污染

研究人员在一个学术中心的急诊科进行了一项前瞻性对照试验,对临床怀疑严重感染并且需要进行血液培养的患者进行标准静脉穿刺取血,来测试这种可以将初始1.5-2 mL血液部分(假定携带污染皮肤细胞和微生物)进行分离的装置(ISDD)是否可以有效预测血液培养污染。

该研究总共纳入了971例受试者,对其中904名非复发性受试者进行了1808例血培养。研究结果显示,与标准程序相比,使用初始标本分流装置(ISDD),血液培养污染明显减少:(2/904 [0.22%] ISDD vs 16/904 [1.78%]标准做法,P = 0.001)。另外,其灵敏度没有受到影响:65/904(7.2%)ISDD与69/904(7.6%)的标准程序检查中确诊出了真性菌血症,P = .41。实验中没有针刺损伤或潜在的血源性病原体暴露的报告。除此之外,使用泊松回归模拟所有护士抽取血液培养物的每月血培养污染的比例,将12个月的干预期与前后6个月的数据进行比较。结果显示:经验丰富的采血者(使用ISDD)血培养污染显着减少,普通护士(没有使用ISDD)血培养污染没有减少。并且,73%的采血工作人员完成了一项匿名调查研究,大多数使用者感觉ISDD装置比较令人满意。

研究人员表示,在急诊科室,使用ISDD可以有效降低血液培养患者的血培养污染。
