Diabetes Care:1型糖尿病与饮食失调的相关性

1型糖尿病患者发生饮食失调的风险更高;然而,对于兄弟姐妹患有1型糖尿病和被诊断为饮食失调之间的关系存在相互矛盾的证据。糖尿病卫生保健团队应该警惕患有1型糖尿病的儿童和青少年的饮食失调行为。
近日,糖尿病领域权威杂志Diabetes Care上发表了一篇研究文章,研究人员旨在明确家庭饮食失调与儿童期发病1型糖尿病之间的相关性和共聚性。
研究人员采用了来自瑞典(n=2517277)和丹麦(n=1825920)国家登记中心的人群样本,调查了1型糖尿病和饮食不规律之间的相关性,以及同卵兄弟姐妹之间的家族聚集性。
根据临床诊断,研究人员将饮食失调分为任何饮食失调(AED)、神经性厌食症(AN)和非典型性AN和其他饮食失调(OED)。研究人员从Cox回归分析中确定了伴有95%CIs的风险比(HRs)评估的相关性。
瑞典和丹麦的1型糖尿病患者被诊断为饮食失调的风险更大(HR[95%CI]瑞典:AED为2.02 [1.80-2.27],AN为1.63 [1.36-1.96],OED为2.34 [2.07-2.63];丹麦:AED为2.19(1.84-2.61),AN为1.78(1.36-2.33),OED为2.65[2.20-3.21])。研究人员还对结果进行Meta分析:AED为2.07 (1.88-2.28),AN为1.68 (1.44-1.95),OED为2.44(2.17-2.72)。在瑞典队列中,同卵兄弟姐妹被诊断为饮食失调的风险增加(AED为1.25 [1.07-1.46],AN为1.28 [1.04-1.57],OED为1.28 [1.07-1.52]);但是这些结果在丹麦队列中并不显著。
由此可见,1型糖尿病患者发生饮食失调的风险更高;然而,对于兄弟姐妹患有1型糖尿病和被诊断为饮食失调之间的关系存在相互矛盾的证据。糖尿病治疗团队应该警惕患有1型糖尿病的儿童和青少年的饮食失调行为。
