JACC:血浆丝氨酸蛋白酶Corin 水平可显著预测急性心肌梗死患者的预后

较低的丝氨酸蛋白酶水平的患者更可能是女性,并有高血压和心脏衰竭(HF)的历史。Kaplan-Meier生存曲线显示,丝氨酸蛋白酶水平在平均数以上的患者与那些丝氨酸蛋白酶水平低于平均数的患者相比,主要不良心脏事件(MACE)的发生率(MACE)和全因死亡率较低。多元回归分析表明,log丝氨酸蛋白酶是MACE的独立预测因子(危险比[HR]:0.61; 95%可信区间(CI):0.42〜0.96; P=0.029),并伴随着年龄的增长,急性心肌梗死史,HF和糖尿病,Killip分级,经皮冠状动脉介入治疗,冠脉搭桥,β-受体阻滞剂的使用,logN-末端B型利钠肽一起进行预测。MACE的C-统计和集成偏差改进通过加入丝氨酸蛋白酶到参考模型中可显著改善。此外,调整临床变量和预后不良的生物标志物建立后,log丝氨酸蛋白酶也被发现是死亡(HR:0.65; 95%CI:0.41〜0.97; P=0.036)和心衰住院(HR:0.48; 95%CI:0.23〜0.90;P=0.009)的显著预测因子。

该研究表明,在AMI患者中,丝氨酸蛋白酶是MACE的一个有价值的预后指标,并独立于传统危险因素。
