对于患有和不患有多囊卵巢综合征(PCOS)的女性,体重指数(BMI)是否会以及如何影响抗穆勒激素(AMH)一直存在争议。了解BMI-AMH之间的关系对实验室检查结果的临床解释具有关键意义,并可能阐明潜在的卵巢生理学机制。近日,内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章,该研究旨在检验一个假设:(1)BMI与PCOS和排卵对照(OVA)女性AMH降低有关,(2)稀释效应不能解释AMH水平的降低。
该研究为多中心队列研究,参加者来自两个临床人群25-40岁年龄段的女性:包含640名PCOS,921名排卵对照(OVA)者。该研究的主要观察指标为卵巢储备指数:AMH;窦卵泡计数(AFC);以及AMH与AFC的比值(AMH/AFC)。
在多因素回归模型中调整了年龄、种族、吸烟和区域后,在这两个队列中,BMI和腰围(WC)的增加与AMH和AMH/AFC的减少有关。BMI的增加与PCOS患者AFC的减少有关,但与OVA无关。加入人体表面积(BSA),与BMI不一样,其与血浆体积成正比,以探究人体大小对AMH浓度潜在的稀释作用。在控制了BSA之后,在两个队列中BMI都保留了与AMH之间的独立关联。BSA与AMH之间不再具有关联。
