JAHA:T波形态恢复预测慢性心力衰竭患者心源性猝死!

慢性心力衰竭患者是心源性猝死(SCD)的高险人群。增加复极离散度已被证实与SCD相关,研究人员推测这应该通过T波的形态及其心率变化来反映。近日,心血管疾病领域权威杂志JAHA上发表了一篇研究文章,该研究的目的是提出一个以心电图(ECG)为基础的指数表征T波形态恢复(TMR),并评估其与慢性心脏衰竭患者人群SCD风险之间的相关性。

研究人员对来自于MUSIC研究的651例接受Holter ECGs检查的慢性心力衰竭患者进行分析研究。TMR是通过使用时间-翘曲度量来测量每RR增量的T波形态变化来进行量化,并且与临床变量,如左心室射血分数和其他心电图-衍生指数,如T波电交替、心率变异性,来比较其预测能力。

研究人员发现在SCD患者中TMR明显高于其他患者(中位数为0.046 vs. 0.039,P<0.001)。当对TMR根据TMR=0.040进行二分类,TMR≥0.040组患者SCD发生率明显升高(P<0.001)。Cox分析显示,TMR≥0.040与SCD密切相关,风险比为3.27(P<0.001),独立于临床和心电图衍生变量。TMR与泵衰竭死亡之间无相关性。
