Radiology:血管形变映射可定量评估胸主动脉瘤患者主动脉变化

VDM为CT监测胸主动脉瘤患者提供了可靠、全面的三维主动脉生长和生长形态定量评估。

胸主动脉瘤(TAA)患者的主动脉直径测量显示有很大的差异。目前还没有一种技术能够以三维(3D)的方式量化主动脉的生长。

血管形变映射(VDM)使用可变形图像配准技术对主动脉生长进行3D评估。这种方法使用高空间分辨率和容积CT血管造影数据,允许在主动脉壁上的所有点全面量化主动脉生长,避免手动定义测量平面的限制。尽管有这些优点,但由于未优化的配准参数(例如,正则化和相似性度量)或降低图像质量的因素(例如,运动或条纹伪影),可能会出现配准错误,进而导致VDM测量错误。因此,需要对TAA患者临床队列中的VDM进行评估,以了解该技术的可靠性和临床实用性。

Nicholas S. Burris等回顾性研究了2006年至2020年期间进行了两次或两次以上CT血管造影研究的TAA患者。使用血管形变映射(VDM)对三维主动脉生长进行量化,这是一种使用变形配准技术来扭曲由基线主动脉解剖构造的网格的技术。比较VDM和临床CT直径测量之间的生长评估。主动脉生长以各网格单元表面积变化的比值(area ratio)来量化。人工分割由独立的评分者进行,以评估评分者之间的重现性。使用手动放置的地标来评估注册错误。使用Pearson相关性和Cohen κ系数评估VDM和临床直径测量之间的一致性。

共有38名患者(68个监测间隔)接受了评估(平均年龄69岁69[标准差];21名女性),主动脉TAA26例、降主动脉TAA10例,升降主动脉TAA2例。VDM在38例患者中有35例(92%)在技术上成功,在68个监测点中有58次(85%)在技术上成功。中位配准误差为0.77 mm(四分位间距为0.54–1.10 mm)。

主动脉分段(Dice相似系数=0.97 6 0.02)和VDM衍生面积比(偏差=0.0,一致性极限:−0.03至0.03)。峰面积比值和直径变化之间有很强的一致性(r=0.85,P<0.001)。VDM检测到58次中有14次(24%)出现增长。VDM显示,在14个生长区间中,有6个(36%)生长区间在最大扩张段外生长,而直径测量没有发现增长。
