近日,心血管疾病领域权威杂志JAHA上发表了一篇研究文章,这项研究旨在调查不良儿童经历(ACEs)与成年后冠心病风险(CHD)之间相关性潜在的心理-生物-行为介导因素。
研究对象为5610名英国公务员(平均年龄为55.5;女性占28%),他们来自1997年至1999年期间基线时无冠心病的Whitehall II队列研究。研究人员通过问卷调查收集了ACE数量的回顾性数据(范围为0-8)。
在第5轮评估中的潜在介导因素包括抑郁和焦虑症状、健康行为(吸烟、酒精依赖、睡眠和体育锻炼)以及心脏代谢异常。从第6轮(2001年)至第11轮(2012-2013年)评估中确定了新的冠心病诊断(心肌梗塞、确定型心绞痛、冠状动脉搭桥术或经皮腔内冠状动脉成形术)。Logistic回归评估了随访期间ACEs、潜在介导因素和冠心病之间的关联。研究人员使用介导分析评估了间接效应。
在随访期间,共有566名(10.1%)参与者发生了冠心病。ACE与冠心病发生的可能性增加相关(每个ACE的比值比为1.09;95%CI为1.00-1.19)。通过控制年龄和性别,介导分析发现抑郁症状(间接效应为1.05;95%CI为1.03–1.07)、焦虑症状(间接效应为1.12;95%CI为1.10–1.15)以及更多的心脏代谢异常(间接效应为1.02;95%CI为1.01-1.03)对ACE与冠心病之间的关联存在间接效应。行为因素不是显著的介导因素。
由此可见,抑郁症状、焦虑症状和心脏代谢异常在一定程度上介导了ACEs与冠心病之间的关系。定期筛查和治疗心理疾病和心脏代谢异常的症状可能有助于减轻ACE的长期健康负担。
