Lancet Neurology:氨甲环酸在脑出血患者中的应用(STOP-AUST): 一项多中心、随机、安慰剂对照的2期试验

全球有1800万人患有脑出血后遗症,每年有300万人死于这种疾病,占所有人类死亡的5%。脑出血患者临床预后的主要决定因素是年龄、神经功能缺损、出血原因、出血部位和出血量。其中,出血量是最重要的因素。

全球有1800万人患有脑出血后遗症,每年有300万人死于这种疾病,占所有人类死亡的5%。脑出血患者临床预后的主要决定因素是年龄、神经功能缺损、出血原因、出血部位和出血量。其中,出血量是最重要的因素。与缺血性中风一样,脑出血通常是一个动态过程。氨甲环酸常用于减少急性创伤或月经过多等情况下的出血量。

本研究是在澳大利亚、芬兰、台湾等13个中风中心进行的一项前瞻性、双盲、随机、安慰剂对照的2期试验。如果患者年龄大于或等于18岁,患有符合临床标准的急性脑出血(如格拉斯哥昏迷评分> 7分,脑出血量< 70毫升,未发现或怀疑脑出血的继发原因,在过去的12个月内未发生血栓形成事件,在接下来的24小时内未计划进行手术,且未使用抗凝剂),在CT血管造影术中出现造影剂外渗(即所谓斑点征),并且在症状出现后4-5小时内和CT血管造影术后1小时内可治疗,则符合条件。

患者被随机分配(1:1),在症状出现后4.5h内开始接受1g的静脉注射氨甲环酸10min且在8h内再服用1g或者使用匹配的安慰剂。主要结果是24小时内脑出血增长(>33%相对或>6ml绝对值)。

在2013年3月1日至2019年8月13日期间,本研究将100名参与者随机分配到氨甲环酸组(n=50)或安慰剂组(n=50)。中位年龄为71岁(IQR 57-79),平均脑出血量为14.6ml(7.9-32.7)。两组的主要结果无差异:安慰剂组26例(52%)和氨甲环酸组22例(44%)出现脑出血增加(优势比[OR]0.72[95%CI 0.32-1.59],p=0.41)。两组之间死亡或有血栓栓塞并发症的患者比例没有明显差异:安慰剂组有8例(16%)死亡,而氨甲环酸组有13例(26%)死亡,2例(4%)对1例(2%)有血栓栓塞并发症。所有死亡均未被认为与研究药物有关。

综上所述,尽管这种治疗是安全的且没有增加血栓栓塞并发症,但本研究并没有提供其预防脑出血生长的证据。由于出血增加是急性脑出血发病率和死亡率的主要原因,因此迫切需要新的、安全和有效的治疗方法来阻止持续的出血。
