Neuroradiology:对于急性视神经患者MRI扫描,横轴位和冠状位,你看好哪个呢?

通过与横轴位图像比较,研究人员旨在评估2D基于导航重建扩散加权成像(RESOLVE-DWI)图像在诊断急性视神经炎的准确性以及利用表观扩散系数(ADC)进行分析的可重复性。

本研究共纳入了49例急性视力丧失的患者并进行视神经长轴DWI扫描(该技术采用了读出-分割的方法进行成像)、RESOLVE-DWI。冠状位扫描层厚为3mm,轴位扫描层厚为2mm。以上图像分别有两名神经放射学家进行回顾性评估。通过诊断结果来计算每种扫描方法的灵敏度、特异度和准确度;观察者内和观察者之间的可靠性是通过kappa检验来评估的。 此外,观察之间的ADC值的一致性是通过组内相关系数(ICC),变异系数(CV)和Bland-Altman图进行评估的。利用非配对t检验进行ADC值间比较。

在49例患者的双侧视神经中,有47条临床上视神经阳性的患者和51例临床上视神经阴性的患者。 两位神经放射学专家在冠状位RESOLVE-DWI序列上的敏感性、特异性及准确性分别为85.1/87.2%,90.2/94.12%,87.8/90.8%,横轴位RESOLVE-DWI序列的敏感性、特异性及准确性分别为83.0/85.1%,66.7/76.5%,75.5/79.6%。两观察者间的K值分别为0.710和0.806,横轴位和冠状状RESOLVE-DWI组内K值分别为0.822和0.909(P <0.0001).对于横轴向和冠状状RESOLVE-DWI的ADC测量的可重复性,观察者中的ICC分别为0.846和0.941,CV值分别为7.046和4.810;Bland-Altman图显示冠状状RESOLVE-DWI的组间变异性较小。阳性组ADC值明显降低(P<0.0001)。

本研究表明,与横轴位相比,在冠状位RESOLVE-DWI图像ADC值具有更高的特异性和更好的可重复性,从而表明了冠状位RESOLVE-DWI序列在诊断急性视神经炎患者的可行性。
