Chest:不发热社区获得性肺炎患者菌血症的发生率和预测因素

由此可见,相当部分的细菌性CAP患者并不发热。与高热菌血症或非细菌性肺炎患者相比,这些患者的死亡率增加。因此,发热作为BC必要性指标的相关性值得重新考虑。
尽管血培养(BCs)是检测菌血症的金标准,但是BCs在社区获得性肺炎(CAP)患者中的实用性存在争议。近日,呼吸领域权威杂志chest上发表了一篇研究文章,这项研究旨在描述了CAP和无发热菌血症患者的比例,并确定了预测无发热患者需要进行BCs的临床特征。
CAPNETZ多国队列研究的4349例CAP患者被招募,并确定了细菌血症的发生率,研究人员根据首次与患者接触时的发热情况对患者进行分层。研究人员采用双因素Logistic回归分析确定了发热患者的细菌血症的独立预测因素。
2116例发热患者中有190例(8.9%)细菌性肺炎,2149例无发热患者中有101例(4.7%)细菌性肺炎,23例低温患者中有1例(4.3%)细菌性肺炎。菌血症的发生率随CURB-65得分增加,从CURB-65得分为0的患者的3.5%上升到CURB-65得分为4患者的17.1%。无发热性菌血症患者的28天死亡率最高(9.9%)。肺炎球菌尿抗原检查阳性(调整比值比(AOR)为4.6,95%置信区间(CI)为2.6-8.2),C反应蛋白>200 mg/l(AOR为3.1,95%CI为1.9-5.2)和血尿素氮(BUN)≥30 mg/dl(AOR为3.1,95%CI为1.9-5.3)是独立的阳性预测指标,抗生素预处理(AOR为0.3,95%CI为0.1-0.6)是无发热患者菌血症的独立阴性预测指标。
