Radiology:MR成像涉足新领域--评价COPD患者局部肺通气的价值

本研究共纳入了27例COPD的患者,在吸入常氧氟化气体混合物(全氟丙烷)行屏气19F气体吸入MR成像,在吸入气体混合物后利用自由呼吸动态19F气体廓清MR成像。利用体积缺失百分比(VDP)、廓清时间、呼吸次数和部分通气量(FV)来定性测算局部肺通气量。利用Pearson相关因子和Fisher z转换来比较不同肺功能参数,并利用Bonferroni法进行多重比较。

与19F混合气体廓清参数的中位数和四分位数相比,所有评价的肺功能测试参数均有显着地统计学差异。呼吸次数中位数(r = -0.82; P < .0001)、廓清时间中位数(r = -0.77; P < .0001)与第一秒用力呼气量(FEV 1)百分比有负性线性相关;相关FV中位数与FEV 1具有正性相关(r = 0.86; P < .0001)。早期与晚期比较,所有受试者的VDP中位数由49%(25-75百分位数为35%-62%)降到6%(25-75百分位数为2%-10%; P < .0001)。在气体吸入早期CDP(VDP initial)与FEV1预测百分比(r = -0.74; P = .0028)和FV(r = 0.74; P = .0002)具有显着相关性。与VDP initial 显示的通气缺失区域(5.8% ,[25-75百分位数为4.0%-7.4%]; P < .0001)相比,在通气区域FV中位数明显升高(11.1% [25-75百分位数为6.8%-14.5%])。
