冠心病患者动脉弹性与血清高敏C反应蛋白和基质金属蛋白酶9的相关性

 动脉系统作为一个整体,其结构和功能改变在心血管疾病发生发展中极其关键。近年研究发现动脉弹性可以反映动脉系统整体结构与功能变化,是早期血管病变的特异性和敏感性指标。它的改变早于疾病临床症状和体征出现并且是预测未来心血管事件的敏感指标。动脉弹性减退是多种危险因素对血管壁损害的综合反映[4] ,其中慢性系统性炎症反应是动脉结构与功能的重要影响因素。同时动脉粥样硬化被广泛地认为是一种慢性炎

动脉系统作为一个整体,其结构和功能改变在心血管疾病发生发展中极其关键。近年研究发现动脉弹性可以反映动脉系统整体结构与功能变化,是早期血管病变的特异性和敏感性指标。它的改变早于疾病临床症状和体征出现并且是预测未来心血管事件的敏感指标。动脉弹性减退是多种危险因素对血管壁损害的综合反映[4] ,其中慢性系统性炎症反应是动脉结构与功能的重要影响因素。同时动脉粥样硬化被广泛地认为是一种慢性炎症性疾病。高敏C 反应蛋白(high sensitivi ty C-reactive protein,hs-CRP) 和基质金属蛋白酶9(matrix metalloproteinase-9,MMP-9)是重要的炎症标记物。在糖尿病患者、高血压患者等人群中二者与动脉弹性呈显著负相关。本研究旨在探讨冠心病患者hs-CRP 和MMP-9 与动脉弹性的关系。

1对象和方法
1.1研究对象


研究对象为我院心内科因诊断或治疗目的行冠状动脉造影的住院患者,同意动脉弹性功能测定,共收集301 例。年龄27 ~ 84 岁,平均59.87 ±11.44 岁。冠心病组:结合冠心病易患因素、临床表现、心电图变化及生物化学指标,并经冠状动脉造影确诊的冠心病患者199 例,其中男性162 例,女性37 例;对照组102 例,男性62 例,女性40 例。排除标准:继发性高血压、先天性心脑血管畸形、心脏瓣膜病、心肌疾病、心包疾病、甲状腺功能亢进症、贫血、感染、恶性肿瘤、自身免疫性疾病、严重慢性疾病、脑卒中、脑血管畸形等。

1.2临床资料收集

详细记录年龄、性别、身高、体重,并计算体质指数。详细询问高血压史、糖尿病史、吸烟等。所有入选病例均在清晨空腹时静脉取血,检查血浆总胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白、载脂蛋白A1、载脂蛋白B、脂蛋白(a)、肌苷、空腹血糖、白细胞总数等。

1.3冠状动脉造影及冠心病确定

按照美国心脏学会、美国心脏协会冠状动脉造影指南,采用PHILLIP 公司大型C 臂心血管X 光机进行造影。采用Judkins 法取多部位造影。经2 位冠状动脉介入专家评阅,主要冠状动脉管腔狭窄大于50%确定为冠心病。

1.4大小动脉弹性功能指数测定

采用动脉弹性功能测定仪(CVProfilorDO-2020,HDI 公司,美国) 检测所有研究对象大动脉弹性指数(large arterial elasticity index,C1)和小动脉弹性指数(small arterial elasticity index,C2)。要求室温18~21℃、灯光柔和、环境安静、被检测者清醒。受试者仰卧,以腕部固定装置缚住右手前臂及腕部,带有支架的探头置于右侧桡动脉搏动最强处。仪器内的软件系统自动识别、计算并显示出压力波形和动脉弹性功能数据,包括C1、C2、收缩压、舒张压、脉压、脉率等。每例受试者连续测量3 次,每次间隔4min,取其平均值作为最后的统计值。

1.5双抗体夹心ABC-ELISA 法检测MMP-9 及hs-CRP

所有入选对象行冠状动脉造影穿刺成功时,收集血液约10 mL,室温凝固, 3 000 g 离心力离心10 min,收集血清。分装后-80益冷冻保存待检。采用双抗体夹心亲和素生物素化酶复合物酶联免疫吸附测定(avidin biotin complex enzyme-linked immunosorbent assay,ABC-ELISA)法检测所有血清标本MMP-9 及hs-CRP。MMP-9 及hs-CRP 试剂盒购自R&D 公司。

1.6统计分析方法


计数资料以例数或相对比表示, 计量资料以表示;两组间计量资料比较采用t 检验计数资料比较采用卡方检验;多组均数间比较采用方差分析;相关关系采用直线相关分析;Logistic 分析用于冠心病危险因素分析,多元逐步线性回归分析用于分析动脉弹性改变与多个变量之间的独立线性关系;均以双侧P <0.05 为差异有统计学意义。

2结果
2.1临床基线资料分析


性别、体质指数、高血压史、糖尿病史、吸烟、低密度脂蛋白、脂蛋白(a)、白细胞总数、收缩压、舒张压、脉压、C1、C2、hs-CRP 和MMP-9 在冠心病组与对照组之间差异具有统计学意义(表1)。

2.2冠心病危险因素Logistic 分析
  以冠心病为应变量,以性别、体质指数、高血压史、糖尿病史、吸烟、低密度脂蛋白、脂蛋白(a)、白细胞总数、收缩压、舒张压、脉压、C1、C2、MMP-9、hs-CRP 作为自变量,运用Logistic 逐步回归分析。回归结果显示:性别、吸烟、低密度脂蛋白、C2、高血压史、hs-CRP 是冠心病的独立影响因素(表2)。

2.3冠心病患者动脉弹性与血清hs-CRP 和MMP-9 相关性分析

  将冠心病患者C1、C2 与血清hs-CRP 及MMP-9分别进行直线相关分析。分析结果显示:C1 与hs-CRP 相关系数r 为-0.51(P <0.01);C2 与hs-CRP相关系数r 为- 0.69(P < 0.01)。C1 与MMP-9 相关系数r 为-0.49(P <0.01);C2 与MMP-9 相关系数r 为-0.55(P <0.01)。

2.4冠心病患者动脉弹性影响因素多元逐步回归分析
  分别以C1、C2 为应变量,以性别、年龄、体质指数、高血压史、糖尿病史、吸烟、空腹血糖、高密度脂蛋白、甘油三酯、低密度脂蛋白、脂蛋白(a)、白细胞总数、收缩压、舒张压、脉压、尿素氮、肌苷、hs-CRP和MMP-9 为自变量,采用多元逐步回归分析。以C1 为应变量的回归结果显示:进入回归方程的大动脉弹性影响因素依次为年龄、脉压、收缩压、hs-CRP、低密度脂蛋白、MMP-9(表3)。以C2 为应变量的回归结果显示:进入回归方程的小动脉弹性影响因素依次为年龄、脉压、空腹血糖、hs-CRP、低密度脂蛋白(表4)。

3讨论


  大动脉弹性指数是指舒张期血流容积减少与压力下降之间的比值,又称容量顺应性;小动脉弹性指数是指舒张期血流容积振荡变化与振荡压力变化之间的比值,又称振荡顺应性。C1 和C2 分别反映大动脉与小动脉弹性功能,数值越小表示大动脉与小动脉弹性越差。多种心血管危险因素可致动脉弹性尤其是C2下降,并且动脉弹性指数与包括冠状动脉在内的多处动脉粥样硬化严重程度相关。而且,多项前瞻性队列研究显示动脉弹性指数可以预测未来较长时间的恶性心脑血管事件。本研究结果显示,分别代表大小动脉弹性的C1 和C2 在冠心病组均明显下降,并且C2 是冠心病的独立相关因子。所以,目前多数学者认为动脉弹性指数尤其C2 是预测心血管风险的良好非创伤性预测指标。冠心病的病理基础是动脉粥样硬化,而动脉粥样硬化是一种慢性炎症性疾病.炎症在动脉粥样硬化的发生、发展中起着重要作用。越来越多的研究结果证实,局部和全身炎症贯穿动脉粥样硬化病变发生、发展的全过程。本研究发现,与非冠心病组比较,冠心病组hs-CRP、MMP-9 和白细胞计数明显增高。进一步对冠心病危险因素Logistic 分析发现,hs-CRP 是冠心病的独立危险因素。这与之前国内外多数研究结果相似。

  那么动脉弹性与炎症的关系如何呢? 近年来研究发现炎症反应是动脉结构与功能的重要影响因素,各种炎症反应与动脉弹性功能有着重要联系。本实验评估了冠心病患者动脉弹性与血清炎症因子hs-RP及MMP-9 之间的关系。相关分析显示C1 与hs-CRP及MMP-9 分别呈明显负相关,C2 与hs-CRP 及MMP-9也分别呈明显负相关,并且均有统计学意义。而且多元逐步回归分析显示hs-CRP 分别是C1、C2 的独立危险因素;与国内外其他研究结果相近。由此可见机体炎症反应是动脉弹性的危险因子,随着机体炎症水平升高动脉弹性功能逐渐下降。

  炎症致动脉功能异常的机制是复杂而多方面的。简而言之,炎症因子诱导内皮活化导致一氧化氮生物利用度下降。一氧化氮是动脉内皮功能标志性介质,在调节动脉弹性功能方面发挥着重要的作用。细胞外基质的稳态影响着血管壁的内在特性,进而影响动脉弹性功能。基质金属蛋白酶是内源性蛋白水解酶,这些酶蛋白调节血管壁细胞外基质的胶原、弹力蛋白及其他物质含量。MMP-9 是基质金属蛋白酶家族中的主要成员之一,可以降解血管壁细胞外基质的胶原、弹力蛋白及其他物质,进而影响动脉弹性。

  总之,本实验研究结果提示炎症是动脉弹性的重要影响因素,临床上应针对机体炎症反应改善动脉弹性进而防治心血管疾病。

  美国学者的一项研究表明,肥胖为高敏C反应蛋白(hs-CRP)升高的最重要预测因素。论文于2012年11月5日在线发表于《美国心脏病学杂志》(Am J Cardiol)。   此项研究从前瞻性人群中纳入289例患者。对多种潜在预测因素与hs-CRP的相关性进行评估。对潜在混淆因素进行校正之后,利用双变量和多变量线性回归模型对肥胖、体育活动、抑郁及其他数种潜在因素进行分析。
