Obesity:改善代谢哪家强,隔日禁食还是节食?

美国伊利诺伊大学芝加哥分校运动机能学和营养学系Krista A. Varady博士指出,目前,对于有糖尿病发生风险的人群(如超重或肥胖且有胰岛素抵抗者)来说,隔日进食引起的血糖调节因素变化是否优于热量限制并不清楚。初步发现表明,间歇性禁食可能比每日限制热量摄入更有效降低有糖尿病发生风险成人的胰岛素抵抗。

Varady及其同事在43例基线体质指数(BMI)≥25 kg/m2且存在胰岛素抵抗的成人中观察了空腹胰岛素、稳态模型评估胰岛素抵抗计算的胰岛素抵抗、BMI、脂肪量、瘦体重和其他几个指标在12个月期间的变化。在这12个月中,11例受试者(平均年龄43岁,81.8%为女性)采用隔日进食,17例受试者采用限制热量摄入(平均年龄42岁,76.5%为女性),其余15例保持正常饮食习惯(平均年龄41岁,73.3%为女性)。

为期12个月的研究包括6个月的减重阶段以及随后6个月的体重维持阶段。隔日禁食者必须在第1天摄入所需热量的125%,并在第2天将摄入量降低到所需热量的25%,这种模式在头6个月一直持续,之后比例变为150%和50%。限制热量摄入是指在头6个月将热量摄入限制在能量需求的75%,在后6个月限制在能量需求的100%。

12个月后,隔日禁食组受试者体重较常规方案组下降8%(P<0.0001)。研究者注意到,与未调整饮食者相比,限制热量摄入组受试者体重下降6%(P<0.0001),这一降幅与隔日禁食组相似。

然而,第12个月时,隔日禁食组受试者的空腹胰岛素水平下降52%,限制热量摄入组仅下降14%(P<0.05)。同时,隔日禁食组胰岛素抵抗下降53%,限制热量摄入组下降17%(P<0.05)。

研究者认为,尽管减重效果相似,但与每日限制热量相比,隔日禁食通常能更好地降低胰岛素抵抗。此外,这些影响可能在基线胰岛素抵抗水平较高的受试者中更为明显。
