在二甲双胍使用者和非使用者中比较了糖尿病和CKD患者的全因死亡率、心血管死亡、心血管事件(死亡、因心力衰竭住院、心肌梗死、卒中或心肌缺血)、终末期肾病(ESRD)和肾脏疾病综合征(ESRD或死亡),这些患者参加了用阿兰斯普(达贝平-阿尔法)治疗减少心血管事件试验(TREAT)(NCT00093015)。在对使用者和非使用者进行倾向性匹配后,并在多变量比例危险模型中对结果进行比较。
结果显示,基线时使用二甲双胍的有591人,非使用者有3447人。在倾向匹配的使用者中,二甲双胍使用者的死亡率、心血管死亡率、心血管事件和综合终点的粗发生率低于非使用者,但ESRD略高(4.0% vs 3.6%)。二甲双胍的使用与降低全因死亡率(HR,0.49;95% CI,0.36-0.69)、心血管死亡(HR,0.49;95% CI,0.32-0.74)、心血管综合事件(HR,0.67,95% CI,0.51-0.88)和肾脏疾病综合事件(HR,0.77;95% CI,0.61-0.98)的风险独立相关。与ESRD(HR,1.01;95% CI,0.65-1.55)的相关性不显著。校正全人群分析中,结果定性相似。观察到两例乳酸性酸中毒。
总之,该研究结果表明,二甲双胍在CKD中的使用可能比之前考虑的更安全,并可能降低3期CKD个体的死亡和心血管事件的风险。
