Circulation:吡格列酮能否降低缺血性卒中或短暂缺血发作后的ACS风险?

研究队列由3876名IRIS参与者组成,平均63岁,65%为男性,89%为白人,12%具有冠状动脉疾病史。中位4.8年的随访期,发生了225例急性冠脉综合征事件,包括141例MI和84例不稳定心绞痛发作。MI包括28例(19%)ST段抬高MI。多数MI为1型(94例,65%),其次是2型(45例,32%)。49名患者(35%)中,血清肌钙蛋白为10倍到100倍正常值上限,39名患者(28%)>100倍正常值上限。吡格列酮降低了急性冠脉综合征风险(风险比,0.71;95% CI,0.54~0.94;P=0.02)。吡格列酮也降低1型MI风险(风险比,0.62;95% CI,0.40~0.96;P=0.03),但未降低2型MI风险(风险比,1.05;95% CI,0.58~1.91;P=0.87)。相似的,吡格列酮降低了血清肌钙蛋白>100倍正常值上限患者的大MI风险(风险比,0.44;95% CI,0.22~0.87;P=0.02),但是小MI风险未降低。

结果表明,不合并糖尿病的胰岛素抵抗患者中,吡格列酮降低近期脑血管事件后急性冠脉综合征的风险。吡格列酮在预防特发性1型MI上具有最明显的效果。
