心肌桥与运动期间发生的心律失常和猝死密切相关。QT离散度是异常复极的一种测量方式并且可以预测心律失常。与正常人群相比,我们观察了在运动期间室性心律失常的发生率、静息状态下及运动后的QTd。

背景与目的:心肌桥与运动期间发生的心律失常和猝死密切相关。QT离散度是异常复极的一种测量方式并且可以预测心律失常。与正常人群相比,我们观察了在运动期间室性心律失常的发生率、静息状态下及运动后的QTd。

方法:我们研究了疑似运动状态下心脏彩超显示心脏间隔屈曲的MB病人(Echo-MB group,N = 510),其中这些人又被另外一种检查所确诊(MB group, N = 110),健康对照组(Control group, N = 198),观察病人在静息状态、压力负荷下的心电图特点。

结果:运动诱导的室性期前收缩(PVCs)发生率,疑似MB组和MB组比正常对照组要多。疑似MB组有25个病人(4.9%)和MB组有7个病人出现了运动诱导的非持续性的室性心动过速(NSVT)。两组的QTd基线没有区别。在疑似MB组和MB组中,与基线相比,运动后的QTd显着增大(both p < 0.001)。与非NSVT的病人相比,男性NSVT高发、QTd显着增大。

结论:MB病人,运动诱导的PVCs及NSVT发生率显着增多。运动诱导的QTd显着增大,有NSVT的MB病人QTd甚至更大。MB病人运动可诱导室性心律失常及复极异常。
