Am J Neuroradiol:动脉瘤壁强化的临床意义

最近的研究显示高分辨MR检测到颅内动脉瘤壁的强化是破裂高风险的指标。瘤壁强化常见于不稳定(破裂、症状性或形态进展)的动脉瘤,对于颅内多发动脉瘤患者瘤壁强化可以用于识别到底是哪一个动脉瘤发生了破裂。

最近的研究显示高分辨MR检测到颅内动脉瘤壁的强化是破裂高风险的指标。瘤壁强化常见于不稳定(破裂、症状性或形态进展)的动脉瘤,对于颅内多发动脉瘤患者瘤壁强化可以用于识别到底是哪一个动脉瘤发生了破裂。

2018年9月来自德国的N. Larsen等在AJNR上发表了他们的研究结果,目的在于探讨MR血管壁影像检观察到的动脉瘤壁强化的病理改变。

研究者回顾了动脉瘤夹闭前未破裂MCA动脉瘤的MR血管壁影像,并根据动脉瘤壁有无强化分为三组,1.五强化,2.弱强化,3.强强化。动脉瘤夹闭后对动脉瘤进行病理分析。

共13个动脉瘤。5个动脉瘤显示壁强化。在这5个动脉瘤中,4个动脉瘤髓过氧化物酶染色显示炎症细胞浸润,3个显示新生血管形成,2个可见血管的滋养血管。7个动脉瘤无瘤壁强化;1个仅轻度强化;这8个动脉瘤中无炎性细胞浸润和新生血管的证据,皆未见血管滋养血管。

最终作者认为MR血管壁影像的动脉瘤壁强化与炎性细胞浸润、新生血管形成和血管滋养血管有关。病理学无炎症表现者皆无强化。这个结果支持如下的假设:MR血管壁影像能为动脉瘤风险分层的提供重要信息。
