Hypertension:动脉硬化与认知功能的关系

主动脉僵硬,而非颈动脉僵硬,与较差的认知表现相关,并且这种关联可部分由微血管功能障碍所解释。

认知障碍的潜在机制尚不完全清楚,但可能与动脉僵硬和微血管功能障碍有关。近日,心血管权威杂志Hypertension上发表了一篇研究文章,在基于人群的马斯特里赫特研究中,研究人员调查了动脉僵硬度和认知能力之间的关联,以及评估了这种关联是否由微血管功能障碍所介导。

研究人员纳入了2544名参与者(年龄为59.7岁;51.0%为男性;26.0%为2型糖尿病患者)的横断面数据,使用颈股脉搏波速度和颈动脉扩张系数分别作为主动脉和颈动脉僵硬度的衡量指标。研究人员根据脑小血管疾病的磁共振成像特征、闪烁光诱导的视网膜小动脉和静脉扩张反应、白蛋白尿以及微血管功能障碍的血浆生物标志物(sICAM-1 [可溶性细胞间粘附分子-1 ]、sVCAM-1 [可溶性血管粘附分子-1]、sE-选择素[可溶性E-选择素]和vWF [von Willebrand因子])计算了微血管功能障碍的综合评分。评估的认知领域包括记忆力、处理速度和执行功能。根据这些领域的平均值计算认知功能评分。

较高的主动脉僵硬度(m/s)与较低的认知功能(β为-0.018 SD[95%CI为-0.036至-0.000])相关,独立于年龄、性别、文化程度和心血管疾病危险因素,但较高的颈动脉僵硬度则不然。较高的主动脉僵硬度(m/s)与较高的微血管功能障碍评分(β为0.034 SD[95%CI为0.014至0.053])相关,而较高的微血管功能障碍评分(每SD)与较低的认知功能相关( β为-0.089 SD [95%CI为-0.124至-0.053]。微血管功能障碍解释了主动脉僵硬对认知功能总效应的16.2%。

由此可见,主动脉僵硬,而非颈动脉僵硬,与较差的认知表现相关,并且这种关联可部分由微血管功能障碍所解释。
