Mol Psychiatry:非侵入性脊髓刺激对未接受药物治疗的重度抑郁症成人的影响

脊髓内感受通路(spinal interoceptive pathways, SIP)向大脑传递有关身体状态的持续信息,长期以来的理论框架提出,大脑中的SIP、皮质皮质连接和相关的贝叶斯主动推理内感受过程通过下降投射来调节身体状态。临床和影像学研究的证据表明,大脑中分布式内感受系统内的区域(例如岛叶皮层)整合和处理内感受信号,并参与MDD的病理生理学。然而,SIP的作用及其作为MDD治疗靶点的潜力尚不清楚。SIP是脑-体相互作用回路中至关重要的传入成,包括无髓鞘C纤维和有髓鞘Aδ传入纤维,它们携带体细胞(如疼痛、温度、瘙痒)和内脏感觉信息,通过背根神经节和突触进入脊髓,到达脊髓背角(如I层)的二级神经元。

根据目前的诊断标准(如DSM-5),MDD的核心症状包括悲伤、情绪低落或易激惹、快感缺乏、食欲紊乱、睡眠、和注意力不集中,以及对自己的消极想法和自杀念头。

经皮脊髓直流电刺激(transcutaneous spinal direct current stimulation, tsDCS)是一种新型、低成本、无创的工具,可用于调节人类脊髓功能,并有可能调节SIP。使用电极蒙太奇进行电场(E-field)模拟,其中阳极位于T10或T11棘突水平,阴极位于肩部,表明2.5至3.0 mA之间的电流可有效到达脊髓,电场最大强度在0.47至0.82 V/m范围内。

趋同的理论框架表明脊髓内感受通路在重度抑郁症 (MDD) 中的作用和治疗潜力。这篇文献旨在评估经皮脊髓直流电刺激 (tsDCS) 在 MDD 中的抗抑郁作用和耐受性。

这是一项针对未服药的中度 MDD 成人的双盲、随机、假对照、平行组、试点临床试验。20 名参与者被随机分配(1:1 比例)接受“主动”2.5 mA 或“假”阳极 tsDCS 治疗,胸椎(阳极;T10)/右肩(阴极)电极蒙太奇 3 次/周,持续 8 周。

统计分析方面使用了方差分析模型分析抑郁严重程度 (MADRS) 评分(预先指定的主要结局)和次要临床结局的变化。使用活动的tsDCS参数生成电场模型。

从研究结果来看,与假手术组(n = 9)相比,活动性tsDCS组n = 10)的MADRS评分基线到终点的下降幅度更大,效应量更大(-14.6 ± 2.5 vs. -21.7 ± 2.3,p = 0.040,d = 0.86 )。 此外,与假手术相比,活动性 tsDCS 导致 MADRS“报告的悲伤”项目更大幅度降低(-1.8 ± 0.4 vs. -3.2 ± 0.4,p = 0.012),并且 tsDCS 会议前/后舒张压从基线到终点的变化累积下降幅度更大(组差异:7.9 ± 3.7 mmHg,p = 0.039)。 MADRS“悲观思想”项目和第 8 周 CGI-I 分数观察到同一方向的统计趋势。在不良事件 (AE) 方面未观察到组间差异,也没有发生严重的 AE。

电流模拟显示,在神经调控范围内(最大~0.45 V/m)内,电场强度达到胸椎脊髓灰质。

总的来说,这项初步研究的结果表明,tsDCS 是可行的、耐受性良好的,并且在 MDD 中显示出治疗潜力。这项工作还为在心理健康研究和治疗背景下谨慎探索非侵入性脊髓神经调控提供了初步框架。其潜在机制值得进一步研究。
