信达生物PD-1单抗霍奇金淋巴瘤关键II期研究数据公布:客观缓解率近80%

5月17日,信达生物公布了其PD-1单抗信迪利单抗(IBI308)用于治疗复发/难治性霍奇金淋巴瘤治疗的关键性ORIENT-1研究的结果。

ORIENT-1研究是一项评估信迪利单抗治疗复发或难治性经典型霍奇金淋巴瘤(cHL)的疗效和安全性的多中心、II期研究,共入组96例复发或难治性cHL患者,是迄今为止中国入组人数最多的复发或难治性cHL研究。

该研究的主要研究终点为客观缓解率(ORR),定义为肿瘤评估的最佳疗效为部分缓解和完全缓解的受试者的比例,采用2007年国际工作小组修订的恶性淋巴瘤疗效评价标准(IWG 2007)作为主要疗效评价标准,由独立中心影像评估委员会(IRRC)进行评价;研究的次要终点是完全缓解率(CR),由于该研究规定所有CR都需要PET进行确认,而基线后的PET检查仅在15周时进行,因此ORIENT-1研究的CR结果仅代表信迪利单抗在15周时的完全缓解情况。

IRRC对96例患者的24周临床数据评估后发现,客观缓解率(ORR)为79.2%,疾病控制率(DCR)为97.9%,达到预设的统计学目标。根据不同标准划分的亚组人群分析结果均显示出相似的客观缓解率;中位缓解持续时间尚未达到,绝大部分患者依然处于持续疾病缓解。研究过程中未发现非预期的安全性问题,无死亡事件发生,信迪利单抗的安全性特征与国外同类产品相似。

美国临床肿瘤学会(ASCO)网站已于2018年5月16日公布了ORIENT-1研究的初步结果,24周的最后详细分析结果将于6月4日以壁报(Poster)形式在ASCO会场进行公布。

信迪利单抗是一种全人源细胞程序性死亡-1(Programmed Cell Death 1,PD-1)单克隆抗体,可以与T细胞表面的PD-1受体结合,阻断其与配体PD-L1之间的结合,使T细胞发挥正常作用,进而利用自身免疫将肿瘤细胞消灭。信迪利单抗由信达生物和礼来在中国共同合作开发,其用于治疗霍奇金淋巴瘤的中国上市申请于2018年4月19日正式获得CDE承办受理,于4月23日被CDE纳入优先审评。信迪利单抗是信达生物首个递交上市申请的药物。

霍奇金淋巴瘤是一种较为罕见的B细胞恶性淋巴瘤,多发生于20-40岁的年轻人。尽管早期HL治疗效果较好、治愈率较高,但许多患者由于治疗不规范、不良反应等原因,一线治疗后仍然会有约20%的复发率。针对复发和难治性霍奇金淋巴瘤,由于治疗方案少,肿瘤内科和血液科医生仍面临严峻的挑战,造血干细胞移植治疗失败后的病人平均存活期只有1~2年左右。Opdivo和Keytruda均已获批治疗经典型霍奇金淋巴瘤的适应症。
