CLIN CANCER RES：通过呼吸道基因表达检测肺癌癌前病变

肺癌是美国癌症相关死亡的主要原因。疾病发生前的分子机制研究很少，并且尚无有效的工具可以鉴别存在可进展为侵袭性肿瘤的癌前病变的吸烟者。之前的研究表明吸烟暴露呼吸道损伤的分子改变与肺癌有关。CLIN CANCER RES近期发表了一篇文章，关注肿瘤进展前的呼吸道损伤以研究癌前病变和其在肺癌预防中的作用。

肺癌是美国癌症相关死亡的主要原因。疾病发生前的分子机制研究很少，并且尚无有效的工具可以鉴别存在可进展为侵袭性肿瘤的癌前病变的吸烟者。之前的研究表明吸烟暴露呼吸道损伤的分子改变与肺癌有关。CLIN CANCER RES近期发表了一篇文章，关注肿瘤进展前的呼吸道损伤以研究癌前病变和其在肺癌预防中的作用。

作者收集了存在癌前病变（n=50）和不存在癌前病变（n=25）表观正常的支气管粘膜的支气管上皮细胞进行测序，鉴别两种人群间基因、通路和肺癌相关基因差异表达。研究发现与癌前病变存在有关的在呼吸道存在的280个基因。在上调的基因中，氧化磷酸化显着增加，免疫组化和生物能学研究确认了癌前病变的通路。利用公共肺癌数据库确认了癌前病变与鳞状细胞癌之间的关系。分子标志可以有效地预测癌前病变的存在（AUC=0.92，n=17），分子标志评分的改变与癌前病变进展或退化有关。

文章最后认为，有癌前病变的吸烟者呼吸道转录学变化反应了代谢及早期肺鳞状细胞癌改变，可以用来鉴别癌前病变进展风险高的吸烟者以及监测肿瘤预防试验的结果。
