J Cosmet Dermatol:多模态CO2和1570 nm激光治疗面部萎缩性痤疮瘢痕

多模态CO2和1570 nm激光治疗可改善面部萎缩性痤疮瘢痕。

瘢痕是寻常痤疮最常见的长期并发症之一,可造成相当大的美容、心理和社会负担,并对患者生活质量产生负面影响其治疗方式复杂多样。最近上市的自动混合点阵激光系统集成了可调谐的CO21570 nm激光,可单独、同时或序贯操作。

J Cosmet Dermatol最新的一篇研究报告,研究者评估了序贯多模态剥脱10600 nm和非剥脱1570 nm激光治疗对痤疮瘢痕的影响。

20 ~ 50岁、Fitzpatrick皮肤II~IV型、无活动性痤疮、寻求改善痤疮瘢痕外观的成年患者被纳入研究。每例患者间隔2个月接受3次多模态激光治疗。治疗前,治疗区域进行彻底清洗和消毒,治疗使用ProScan Hybrid applicator (Alma Laser Ltd, Caesarea, Israel) CO2激光 (45 W, 1 ms)和1570 nm激光(10 W, 3 ms)。在整个治疗过程中,使用外部空气冷却装置(Cryo 6, Zimmer Aesthetics , Germany)对皮肤进行冷却。未使用表面麻醉药。此外,受试者每天至少涂3次修复霜,在阳光下每2小时涂一次防晒系数50+的防晒霜。在基线和最后一次治疗后6个月评估瘢痕体积和受累面积。采用标准5Likert量表进行患者满意度评分。

16例患者进行剥脱联合点阵非剥脱激光治疗痤疮瘢痕。男性9例,女性7例,平均年龄29.3±7.9岁。Fitzpatrick皮肤分型:III型11例(68.8%),II型4例(25%),IV型1例。基线时,所有患者均有箱车型和滚轮型瘢痕,仅3例患者有冰锥型瘢痕。治疗后6个月,平均瘢痕体积从基线时的5.7±5.2 mm3改善至3.1±3.0 mm3,平均受累面积从165.6±134.0mm2改善至94.0±80.1mm2,分别较基线减少47.0±7.9%和43.2±8.6%。患者对治疗效果满意度高,治疗过程中及随访期间均未发生严重不良反应。

综上,多模态CO21570 nm激光治疗可改善面部萎缩性痤疮瘢痕。根据美容区域、瘢痕特征和皮肤光型定制治疗间隔和激光参数可能进一步提高治疗效果,并扩大其对其他皮肤问题的适用范围。
