Blood:蛋白质表达谱表明孤立性肺栓塞中非经典途径的相关性

基于其急性血浆蛋白质组,孤立的 PE 与 DVT 相关的 PE 有很好的区别,表明其具有独特的病理生理学。在基于人群的队列的外部验证中,大多数孤立的 PE 相关蛋白预测了原发性孤立性 PE 的发生。

孤立性肺栓塞 (PE) 患者的临床特征与深静脉血栓形成 (DVT) 相关的 PE 患者不同,后者有更多的肺部疾病和动脉粥样硬化。这些发现表明了独特的分子病理生理学和替代途径在孤立性 PE 中的潜在参与。

为了验证这一假设,分析了来自静脉血栓栓塞基因分型和分子表型项目的 532 名个体的数据,该项目是一项具有广泛生物样本库的多中心前瞻性队列研究。

基于其急性血浆蛋白质组,孤立的 PE 与 DVT 相关的 PE 有很好的区别,表明其具有独特的病理生理学。在基于人群的队列的外部验证中,大多数孤立的 PE 相关蛋白预测了原发性孤立性 PE 的发生。

研究方法:应用靶向、高通量蛋白质组学、机器学习和生物信息学方法,将孤立性 PE 患者(n = 96)的急性期血浆蛋白质组与 DVT 相关性 PE(n = 276)或孤立性 DVT 患者的急性期血浆蛋白质组进行对比。 n = 160)。

这导致鉴定了 PE 表型之间共享的分子过程,以及分离的 PE 特异性蛋白质特征。共同的过程包括炎症的上调、对氧化应激的反应和肺表面活性剂的损失。分离的 PE 特异性特征由 5 种蛋白质组成:干扰素-γ、神经胶质细胞系衍生的神经营养生长因子、多肽 N-乙酰半乳糖胺基转移酶 3、肽基精氨酸脱亚胺酶 2 型和白细胞介素 15 受体亚基 α。这些蛋白质使用顺式蛋白质数量性状基因座进行正交验证。

在一个独立的基于人群的队列 (n = 5778) 中进行的外部复制进一步验证了蛋白质组学结果,并表明它们是无 VTE 病史的个体中发生原发性孤立性 PE 的预后因素(中位事件发生时间:2.9 年;四分位距:1.6- 4.2 年),支持他们可能参与早期发病机制。这项研究确定了 VTE 表型之间的分子重叠和差异。

特别是,结果表明在孤立性 PE 的急性病理生理学中,非经典途径更常与呼吸和动脉粥样硬化疾病相关。
