肝细胞癌(HCC)是我国最常见的恶性肿瘤之一,乙型肝炎病毒(HBV)感染和肝硬化(LC)是最主要的癌变因素,全球三分之一的患者集中在我国,发病率高,同时伴随较高的死亡率,五年总生存率不足50%。

如何实现早期诊断,更早介入治疗并推进准确的预后预测,是肝癌患者得到个性化治疗的关键所在,也是全球相关领域的研究热点。

2021年1月25日,欧洲肿瘤学会杂志ESMO Open刊载了由福建医科大学孟超肝胆医院、四川大学华西医院疾病分子网络前沿医学中心、西南医科大学附属医院、泰莱生物研究团队联合发表的科研成果论文《结合血浆细胞游离DNA的5-羟甲基胞嘧啶特征和蛋白质生物标志物进行肝细胞癌诊断和预后的液体活检》(附英文全称:Liquid biopsy by combining 5-hydroxymethylcytosine signatures of plasma cell-free DNA and protein biomarkers for diagnosis and prognosis of hepatocellular carcinoma)。

 

本项研究共纳入362名参与者(165名健康志愿者、62名肝硬化患者、135名肝癌患者)。同时,研究共收集了347份肝癌患者的血浆样本,其中包括135例术前患者样本、105例术后患者样本,剩余的107份血浆样本来自长期跟踪的10位肝癌患者。研究团队对其游离DNA(cfDNA)进行基于选择性化学标记的羟甲基化高通量测序,通过最小偏二乘鉴别分析和交叉验证,建立了高敏感度的胞嘧啶的羟甲基化(5-hydroxymethylcytosine, 即5hmC)诊断模型,以定义加权诊断评分(wd-score),再将其与蛋白标志物结合建立更稳健的肝癌诊断评分体系。

研究显示,不同5hmC区的分布模式可明确地区分肝癌患者、肝硬化患者和健康人群。在肝癌与非肝癌人群的区分上,基于cfDNA中64个5hmC信号的wd-score达到93.24% AUC(曲线下面积);而在肝癌与肝硬化患者的区分上,结合蛋白标志物时,评分可达92.75% AUC。同时,研究团队建立的该诊断体系对手术后高复发风险患者的筛查能力很强,可作为无复发生存率和总生存率的独立参考指标,且有明确的统计学意义。此外,研究发现肝癌患者血浆样本中,诊断体系评分的分值与随访期间的肿瘤负荷动态呈正相关。

本项研究证明,通过cfDNA中5hmC修饰联合血清学蛋白质标志物AFP、DCP,可切实建立高效的肝细胞癌无创诊断方法

本项研究成果通讯作者,福建医科大学孟超肝胆医院肝病副院长刘小龙教授表示:“我国是肝癌大国,肝硬化是我国原发性肝癌最主要的并发源。多年来,肝硬化和肝癌的临床鉴别诊断仍然依赖影像学、组织活检和甲胎蛋白(AFP)等肿瘤标志物的长期跟踪检测。不过,受限于常规检测手段的特异性和灵敏性,70%的患者在确诊时就已经失去了手术机会,所以整体预后仍然很差。在本项研究中我们建立的诊断评测体系中,肝癌诊断评分的分值与随访期间患者的肿瘤负荷动态呈现正相关;不仅如此,在鉴别肝癌和肝硬化患者的模型训练和测试中,AUC也分别达到92%和95%,这都将有助于我们准确筛选肝癌高危人群,对临床医生来说,具有辅助优化肝癌治疗过程中临床决策的意义。”

本研究的另一位通讯作者,四川大学华西医院疾病分子网络前沿医学中心研究员、泰莱生物顾问科学家谢丹教授介绍:“这项研究开发并验证了新的无创液体活检策略,将加权5hmC诊断模型、AFP和HCC评分相结合,进一步构建了新的肝癌诊断体系,用于肝癌的诊断和预后以及肝硬化-肝癌进展的监测。与传统的标志物检测相比,该诊断模型可克服肝癌患者高度异质性,具有更高的敏感性和稳固性。接下来,我们还将持续扩大本项研究的对象人群,以对其大规模随机临床试验进行进一步验证”

“与传统的影像检测和组织活检相比,液体活检技术可更便捷、更频繁、更动态地进行应用监测。此外由于检测cfDNA中的5hmC信号只需在共价化学标记的基础上进行全基因组范围测序,具有低测序偏差和成本效益的优势,如应用于常规的临床检测场景,其价格和检测体验也更容易被患者接受,临床应用潜力巨大。”本文作者,泰莱生物医学研究与拓展副总裁钟晟博士介绍道:“此前我们基于本研究方法,已在胰腺癌等恶性肿瘤的筛查和诊断方面有突破性进展,相关的研究成果可为癌症手术、治疗的规划提供辅助诊断依据,或将成为临床传统检查手段之外的重要补充。”

泰莱生物在肿瘤单病种临床诊断方面拥有多条产品研发管线。此前,由泰莱生物顾问科学家谢丹教授,以及四川大学华西医院胰腺外科副主任李昂教授领衔的突破性成果《游离DNA表观遗传标志物联合模型用于胰腺癌诊断》,于2020年7月被世界权威表观遗传学研究期刊Clinical Epigenetics刊载。该研究证明利用cfDNA中的5mC联合5hmC可建立高效的胰腺癌无创诊断方法,团队利用基于游离甲基化DNA免疫沉淀的高通量测序技术对164例血浆样本中的cfDNA进行了甲基化研究,其中包括67例胰腺导管癌和97例拮抗对照,并使用wd-score数值化评分模型进行分析对照。

该模型下计算的wd-score不仅能区分胰腺癌和健康人样本,还可实现不同分期(早、晚期)的划分,5hmC标志物对早期胰腺癌诊断具有优势。此外,双组学模型在预测灵敏度方面,比基于5mC或5hmC的单组学预测模型提高约10%,可明显提高胰腺癌的诊断效力尤其在对胰腺癌早期诊断的灵敏度上,预测精度提高尤为突出。
