JAHA:早发性先兆子痫与糖萼降解和微血管灌注减少有关

由此可见,糖萼降解和微血管灌注减少与早发型先兆子痫中的内皮功能障碍和活化以及血管损伤有关。

内皮糖萼是血液和内皮之间的血管保护屏障。近日,血管疾病领域权威杂志JAHA上发表了一篇研究文章,研究人员假设糖萼降解存在于先兆子痫中,这是一种以内皮功能障碍和激活为特征的妊娠特异性高血压疾病。

研究人员使用侧流暗视野成像无创地检查了正常妊娠期(n=73)、早期(n=14)或晚期(n=29)发病的先兆子痫或妊娠期糖尿病(n=21)孕妇的舌下糖萼)。研究人员计算了红细胞可渗透的糖萼的宽度(称为灌注边界区域,糖萼降解的指标)和充满红细胞的血管百分比≥50%的时间(微血管灌注的指标)。此外,研究人员通过ELISA测量了参与者亚组中糖萼组分的循环水平,包括硫酸乙酰肝素蛋白多糖、透明质酸和SDC1(多配体聚糖1)。研究人员通过重复测量的ANOVA以调整血管直径和咖啡因摄入量。

患有早发性先兆子痫的妇女表现出较高的糖萼降解,表现为较大的灌注边界区域(平均值为2.14 [95%CI为2.05-2.20]),相比于其它组(正常血压平均值为1.99 [95%CI为1.95-2.02] ],P=0.002;迟发性先兆子痫:2.01 [95%CI为1.96-2.07],P=0.024;妊娠期糖尿病:1.97 [95%CI为1.91-2.04],P=0.004)。在早发型先兆子痫中,充满红细胞的血管百分比显著降低。与正常血压妊娠相比,在早发型先兆子痫患者中这些结构性糖萼变化伴随着糖萼组分,硫酸乙酰肝素蛋白多糖和透明质酸的血浆浓度升高。

由此可见,糖萼降解和微血管灌注减少与早发型先兆子痫中的内皮功能障碍和活化以及血管损伤有关。
