Circulation:骨髓内皮细胞对糖尿病骨髓造血的调节作用

糖尿病是一个常见公共健康问题,在中国,约有1.1亿糖尿病患者。糖尿病可导致严重的心血管并发症,明显增加冠心病的风险。骨髓造血在糖尿病并发症发生中具有怎样的作用,目前尚不明确。

糖尿病是一个常见公共健康问题,在美国,约1/3的人罹患糖尿病,在中国,约有1.1亿糖尿病患者。糖尿病可导致严重的血管并发症,明显增加冠心病的风险。骨髓造血在糖尿病并发症发生中具有怎样的作用,目前尚不完全清楚。

近期,《Circulation》杂志上发表了Hoyer等骨髓内皮细胞在糖尿病炎性骨髓细胞产生调控中的作用的研究。

在该研究中,研究人员检测了3种糖尿病小鼠(分别是由链脲佐菌素、高脂饮食和敲除瘦素受体[db]诱导的)的白细胞、造血干/祖细胞(HSPC)和骨髓内皮细胞。

在糖尿病患者中,HSPC增殖增强,导致循环髓系细胞数量增多。骨髓生态位细胞分析显示,糖尿病小鼠内皮细胞Cxcl12的表达较少,Cxcl12可促进HSPC静止。对骨髓内皮细胞进行转录组分析表明,在饮食诱导的糖尿病小鼠中,与上皮生长因子受体(EGFR)信号相关的基因富集。


为了探索内皮Egrf是否在骨髓生成中起功能作用,Hoyer等建立了Egfr内皮特异性缺失的小鼠 (Cdh5CreEgfrfl/fl)。结果发现,与糖尿病野生型小鼠相比,Cdh5CreEgfrfl/fl小鼠的HSPC增殖和髓系细胞生成均增加。内皮细胞EGFR信号中断降低了HSPC保留因子血管生成素-1的表达。


Hoyer等还检测了这些发现与伤口愈合和动脉粥样硬化(都与糖尿病并发症有关)的功能相关性。在糖尿病Cdh5CreEgfrfl/fl小鼠的皮肤伤口中,炎性髓样细胞积聚较多,明显延迟了伤口的愈合。Cdh5CreEgfrfl/fl小鼠的动脉粥样硬化加速,导致主动脉中出现更多更大的炎性动脉粥样硬化斑块。

综上所述,糖尿病患者骨髓内皮细胞参与骨髓造血功能紊乱。糖尿病减少了内皮产生CXCL12,CXCL12是一种促进HSPC静止的生态位因子,可以减少干细胞的增殖。本研究描述了一个既往未知的反馈调节途径,保护性的内皮EGFR信号抑制了HSPC的增殖和髓样细胞的产生。
