Brian:常见的脑内病变——第一次脱髓鞘时大脑发生了什么变化?

这两种先进的MRI技术在检测早期MS的临床相关病变方面更加敏感,可以提前对MS患者病情进行预测。

脱髓鞘疾病是一类病因不相同、临床表现各异、但有类同特征的获得性疾患,其特征的病理变化是神经纤维的髓鞘脱失而神经细胞相对保持完整。髓鞘的作用是保护神经元并使神经冲动在神经元上得到很快的传递,所以,髓鞘的脱失会使神经冲动的传送受到影响。

其中,多发性硬化(MS)是最常见的脱髓鞘疾病。常见的症状包括复视、单侧视力受损、肌肉无力、感觉迟钝,或协调障碍。多发性硬化症的病情多变,患者的症状可能反复发作,也可能持续加剧。在每次发作之间,症状有可能完全消失,但永久性的神经损伤仍然存在,这在病情严重的患者特别明显。

在早期多MS中,更清楚地了解正常脑组织的微观结构和代谢异常将为其病理生理学提供宝贵的见解。为此,来自英国伦敦大学神经影像学的专家使用多参数定量MRI检测第一次脱髓鞘发作的患者的脑组织的改变,以期获得神经元定向分散和密度成像和23Na MRI(以估计总钠浓度,反映代谢功能的潜在变化)。相关结果发表在《大脑》(Brain)杂志上。

在这项横断面研究中,研究人员招募了42名在第一次脱髓鞘事件发生后3个月内被诊断为临床孤立综合征(CIS, 出现中枢神经系统炎性脱髓鞘的单发发作)或MS的患者及16名健康对照组。对所有参与者进行人之量表评估,并获得大脑和脊髓3T结构扫描,以及神经元定向散布和密度成像。同时,32名患者和13名健康对照组也接受了脑部23Na磁共振成像以测量大脑正常外观的白质、白质病变和灰质中的神经元密度和定向分散指数(ODI)以及总钠浓度。

结果显示,与健康对照组相比,患者在包括胼胝体在内的外观正常的白质中表现出较高的ODI与总钠浓度,而相应部位的神经元密度则较低。在灰质方面,与健康对照组相比,患者表现出额叶、顶叶和颞叶皮层的ODI较低;顶叶、颞叶和枕叶皮层的神经元密度指数较低;边缘和额叶皮层的总钠浓度较大。

脑容量在患者和对照组之间没有差异。在患者中,胼胝体中较高的ODI与定时行走能力较好有关,而与大脑和病变体积无关。左额中回的总钠浓度较高与扩展残疾状态量表的残疾程度较高有关。

在外观正常的白质,特别是胼胝体中发现轴突分散度(axonal dispersion)增加,同时存在轴突变性和总钠积累。胼胝体中轴突分散度的增加与定时行走能力的恶化之间的联系意味着这一结构的形态和代谢改变可能在机制上导致MS患者的运动能力障碍甚至残疾的发生。

综上,该研究表明,这两种先进的MRI技术在检测早期MS的临床相关病变方面更加敏感,可以提前对MS患者病情进行预测。
