JAHA:血管钙化是非老年人肾功能恶化的危险因素

高CACS患者肾功能下降更快,提示血管钙化可能与慢性肾脏疾病进展相关
血管钙化与慢性肾脏疾病之间的关系众所周知。然而,血管钙化是否影响肾功能恶化尚未明确。
近日,心血管疾病领域权威杂志JAHA上发表了一篇研究文章,研究人员旨在评估冠状动脉钙评分(CAC)确定的较高的血管钙化程度个体肾功能恶化是否更快。
研究人员回顾性地分析了2010年1月至2012年7月期间在韩国天安市一所高等教学医院接受心脏计算机断层扫描且肾小球滤过率(>60 mL/min/1.73m2)正常的患者。所有参与者的年龄在为20-65岁。
肾脏结局风险比
在739例患者中,CACS为0、1-99、≥100单位的患者分别为447、175和117例。参与者被随访了7.8年(四分位数范围为5.5-8.8年)。与CACS为0分的患者相比,校正后的年度估计肾小球滤过率在CACS≥100组患者中下降更快(校正后‐β为-0.40;95%CI为-0.80至-0.03)。CACS≥100的患者肾脏疾病:改善整体预后标准(肾小球滤过率估计下降25%或更大)的调整风险比为2.52(1.13-5.61)。
倾向评分匹配后,与CAC为0(1.9%)的患者相比,CAC≥100的患者(13.2%)肾脏结局更为常见,差异具有统计学意义(P=0.004)。
由此可见,该研究的结果显示,高CACS患者肾功能下降更快,提示血管钙化可能与慢性肾脏疾病进展相关。
