心脏起搏细胞拥有形成自身电脉冲的“发动机”——“局部钙释放(LCR)”。然而,迄今为止,人们还不清楚该“发动机”的启动机制。近日,中国科学院院士、同济大学附属东方医院教授陈义汉团队在《细胞研究》发文,揭示了正常心跳“发动机”的“点火”装置和“点火”程序,为心律失常防治提供了重要的基础研究数据。

该论文第一作者、同济大学附属东方医院研究员解端阳介绍说,LCR是心脏收缩活动的上游信号,它通过“LCR—心脏起搏细胞发放电脉冲—心肌细胞兴奋—心肌细胞发生钙瞬变—心肌细胞收缩—心脏收缩”信号通路引起心脏收缩。但是,LCR被触发的机制依然是医学领域的认知盲区,从源头上阻碍了临床上心律失常防治技术的进步。

在长期的研究中,该团队观察到心脏起搏细胞内谷氨酸富集区的位置与LCR的发生区域高度重叠,这提示谷氨酸与LCR之间存在着潜在的相关性。为探索谷氨酸在LCR产生中的作用,该团队从起搏细胞外部和内部两个层面操控谷氨酸浓度,观察LCR的变化。

实验结果显示,起搏细胞外液的谷氨酸改变并不能有效影响LCR的动力学;而显微注射技术带来的细胞内部谷氨酸浓度的改变,可以引起局部钙释放频度、振幅、宽度和面积的显著性改变,这说明心脏起搏细胞内部而不是外部的谷氨酸可以调控LCR。

为了确认这一初步发现,研究人员将谷氨酸直接加到经透膜处理的心脏起搏细胞上。结果发现,LCR产生了类似于上述细胞内显微注射谷氨酸带来的变化,由此证明了细胞内谷氨酸确实对LCR行使了“点火”功能。

通过机制研究发现,心脏起搏细胞的线粒体膜上所富集的兴奋性氨基酸转运蛋白1(EAAT1)对谷氨酸介导的LCR变化发挥了关键性作用。EAAT1转运胞浆中的谷氨酸进入线粒体内部,进而促进线粒体产生活性氧,后者氧化钙处理蛋白,最终‘点燃’LCR。

该团队还分别从细胞、器官和整体三个层面证实了EAAT1可以充当窦房结起搏细胞自主节律的调控靶点。未来可以把‘EAAT1—线粒体活性氧—钙处理蛋白氧化—钙处理蛋白’作为潜在的靶标体系,为临床心律失常的防治研发创新药物
