FROG-ICU是在ICU幸存者出院后1年内进行的一项前瞻性观察性多中心队列研究,包含了法国和比利时的21个内科、外科或混合ICU。所有连续入住ICU至ICU出院超过24小时的需要有创机械通气和/或血管活性药物支持的患者被纳入研究。该研究的主要结局指标是ICU出院后1年内的全因死亡率。研究人员测定了受试者ICU出院的临床和生物学参数,包括循环心血管生物标志物B型利钠肽前体N末端、高敏肌钙蛋白I、生物活性肾上腺髓质素和可溶性ST2,并使用验证了的剥夺指数(FDep)进行社会经济状况评估。
在1570例出院的ICU幸存者中,其中333例(21%)幸存者死于第二年。多变量分析确定了年龄、合并症、输入红细胞、ICU住院时间和ICU出院时的常见异常临床因素(低收缩压、体温、总蛋白、血小板和白细胞计数)作为独立因素与1年死亡率相关。当加入多变量模型时,升高的心脏和血管衰竭生物标记物与1年死亡风险独立相关,合并死亡风险几乎增加3倍(校正比值比为2.84(95%可信区间为1.73-4.65),P<0.001)。
由此可见,FROG-ICU研究确定了在ICU出院时潜在的临床和生物学因素,这些因素与ICU出院后的长期不良结局相关,其可以指导出院计划和直接干预。
