JAMDA:NEAT人工神经网络对鉴别易跌倒老年人有效

背景;如何正确识别老年人存在的反复跌倒的风险是复杂而又困难的。为此,我们研究的主要目的是研究三个人工神经网络即MLP、调整后的MLP、NEAT,利用一系列与社区老年人跌倒因素相符合的临床特征,测试它们区分经常跌倒与不经常跌倒老年人的有效性(人工神经网络,简写为ANNs,是一种模仿动物神经网络行为特征,进行分布式并行信息处理的算法数学模型。这种网络依靠系统的复杂程度,通过调整内部大量节点之间相互连接

背景;

如何正确识别老年人存在的反复跌倒的风险是复杂而又困难的。为此,我们研究的主要目的是研究三个人工神经网络即MLP、调整后的MLP、NEAT,利用一系列与社区老年人跌倒因素相符合的临床特征,测试它们区分经常跌倒与不经常跌倒老年人的有效性(人工神经网络,简写为ANNs,是一种模仿动物神经网络行为特征,进行分布式并行信息处理的算法数学模型。这种网络依靠系统的复杂程度,通过调整内部大量节点之间相互连接的关系,从而达到处理信息的目的。人工神经网络具有自学习和自适应的能力)。

研究方式:


我们使用的是横断面设计,招募了来自3289个生活在不同的小区平均年龄在65岁及以上的志愿者。分析指标有年龄、性别、体重指数(BMI)、服用药物的数量、精神药物的使用,体内二膦酸盐和钙含量、维生素D补充剂的使用、是否需要助行器、是否担心摔倒、远视力评分、定时起立-行走得分、下肢本体感受、握力强度、有无抑郁症状和认知障碍及有无跌倒史。依据志愿者在过去一年内有无跌倒及跌倒次数将他们分为两个组:跌倒不超过1次组和跌倒超过一次组。不仅如此,所有的志愿者都分开接受ANNs测试和对照测试。

结果:

在3289名志愿者中,有622名是反复跌倒的(占18.9%),相比MLP和调整后的MLP而言,NEAT利用15种临床特征进行分析更有效果,例如,需要助行器、担心摔倒心态、钙物质摄入量、有抑郁症状、使用维生素D补充剂、女性、认知障碍、BMI <21 kg/m2,日常服用药物超过4种 、视觉评分低于8分、服用精神药物、下肢肢体感觉得分不超过5分、TUG时间超过9秒、握力得分不超过29牛、年龄大于等于75岁),这一工具显示出鉴别经常跌倒老年人的有效性、敏感性(80.42%)、特异性(92.54%)、阳性预测价值(即易跌倒)为84.38、阴性预测价值(即不易跌倒)为90.34、准确性为88.39、 Cohen κ检验效应为0.74。

结论:

结果表明,使用15种临床特征的NEAT人工神经网络对鉴别反复跌倒的社区老年人更加有效。
