Richter转化(RT)是一种侵袭性淋巴瘤,发生于慢性淋巴细胞白血病(CLL)。转化与涉及TP53、CDKN2A、MYC和NOTCH1的CLL期基因异常有关,然而,相当大比例的RT病例缺乏CLL期相关事件。
AKT激活增强与CLL→RT转化增多相关
在该研究中,研究人员发现携带TP53和NOTCH1突变的高危型CLL患者和RT患者的AKT磷酸化水平较高。

在Eμ-TCL1 CLL小鼠模型中,Akt基因过度激活导致CLL转化为RT,显著降低存活率和侵袭性淋巴瘤表型。在缺乏复发性突变的情况下,研究人员鉴定出了介于CLL和DLBCL之间的基因组突变图谱。
Notch1激活促进RT
通过对Akt诱导的小鼠RT的磷酸化蛋白质组/蛋白质组学和单细胞转录图谱的多组学分析发现,由CLL细胞发展而来的高度侵袭性淋巴瘤细胞亚群可由S100蛋白定义,通过T细胞表达的Notch配体激活Notch。组成性活化的Notch1可同样诱导小鼠CLL转化为RT。
综上所述,通过诱导RT细胞和微环境T细胞之间的Notch信号,该研究确认Akt激活是CLL向侵袭性淋巴瘤转化的启动事件。
