J Ethnopharmacol:四妙丸减轻高尿酸血症肾损伤的多组学分析

该研究探讨四妙丸对高尿酸血症的治疗作用,评价其对高尿酸血症所致肾损伤的保护作用,并探讨其作用机制。

背景:四妙丸由黄柏、薏苡仁、苍术和牛膝组成,由于其清热利尿的特性,传统上被用于治疗高尿酸血症。现代药理学研究表明,四妙丸具有抗炎、抗氧化和免疫增强作用,既往研究也发现四妙丸治疗痛风关节炎的抗炎活性优于非布司他,提示研究表明四妙丸在治疗高尿酸血症方面具有很大的潜力。然而,四妙丸治疗高尿酸血症的确切成分及其潜在的药理机制还有待进一步研究。

研究目的:探讨四妙丸对高尿酸血症的治疗作用,评价其对高尿酸血症所致肾损伤的保护作用,并探讨其作用机制。

方法:采用超高效液相色谱-质谱联用技术对四妙丸的成分进行鉴定。通过腹腔注射氧酸钾(PO)和口服次黄嘌呤(HX)建立高尿酸血症小鼠模型。结合网络药理学、转录组学和代谢组学分析,探讨四妙丸降尿酸、保肾的作用机制。进行了机制和功能研究以验证潜在的机制。

结果:四妙丸含有12种特征成分。四妙丸治疗可显著降低血清尿酸水平,改善高尿酸血症所致肾损伤。四妙丸抑制XOD和XDH酶活性,调节肾脏和回肠尿酸转运体。转录组学和网络药理学分析强调槲皮素、黄连素、山奈酚和黄芩素是四苗丸在高尿酸血症治疗期间对肾脏起作用的主要活性成分,主要影响纤维化、细胞凋亡和炎症相关的信号通路。

代谢组学分析揭示了四妙丸抗高尿酸血症肾损伤的21种差异代谢物和5种代谢途径。进一步的实验结果证实,四妙丸可降低肾纤维化、肾细胞凋亡、血清炎症水平,抑制NF-κB/ NLRP3/IL-1β信号通路

结论:四妙丸通过调节炎症、细胞凋亡和肾纤维化,显著降低血尿酸水平,改善肾损伤。这些发现为四妙丸治疗高尿酸血症患者提供了强有力的科学药理学依据。
