Eur Respir Rev:肺动脉高压可进一步降低慢阻肺患者的运动耐量

肺动脉高压(PH)是COPD患者常见的一种并发症,与COPD患者住院风险增加、更差的临床病程和死亡风险增加相关,是否也会影响患者的运动耐量?

慢性阻塞性肺疾病(COPD,简称慢阻肺)是一种常见的、可预防的并可治疗的一种疾病,其特点是持续性气流受限和呼吸道症状,多是由大量吸烟或有害颗粒暴露引起的。

肺动脉高压(PH)是COPD患者常见的一种并发症,与COPD患者住院风险增加、更差的临床病程和死亡风险增加相关。然而,PH在COPD患者中的发生率尚未有确切数据,据报道,发生率波动在23%-91%之间,因诊断标准和COPD的严重程度而不同。

本研究旨在明确在COPD患者中,PH是否会降低其运动耐量。

纳入了采用心肺运动试验(CPET)评估有PH的COPD患者(COPD-PH)和无PH的COPD患者(COPD-nonPH)的运动耐量的研究。两名独立的审查员分析了这些研究,提取了数据,并评估了证据的质量。

两组患者的运动耐量的变量

筛查了4915篇文章,其中有7篇共涵盖了257名COPD-PH患者和404名COPD-nonPH患者。与COPD-nonPH患者组比较,COPD-PH患者组的最大氧耗(VO2peak,-3.09 mL·kg-1·min-1,95%CI -4.74~-1.43,p=0.0003)、最大负荷(Wmax,-20.5 W,95%CI -34.4~-6.5,p=0.004)和氧脉搏(O2脉搏,-1.24 mL/次,95%CI -2.40~-0.09,p=0.03)均存在显著差异

如果排除肺移植患者的研究,敏感性分析则会显示出更大的差异:V'O2:-4.26 mL·min-1·kg-1(95%CI -5.50~-3.02,p<0.00001);Wmax:-26.6 W(95%CI -32.1~-21.1,p<0.00001);O2脉搏:-2.04 mL/次(95%CI -2.92~-1.15,p<0.0001)。

综上所述,合并肺动脉高压的COPD患者的运动耐量明显低于无肺动脉高压的COPD患者,尤其是对于非移植候选患者。
