肿瘤转移后仍有望根治,微创消融技术提供生机

近年来肿瘤治疗逐渐向微创化、精准化转变。微创治疗技术在肿瘤的诊断、治疗、并发症的处理等方面已发挥不可替代的作用。以肿瘤消融治疗为代表的微创技术以“最小损伤人体结构、有效杀灭肿瘤”为原则,具有创伤小、疗程快、疗效突出等优势,是未来肿瘤治疗的重要方向。为进一步推动和推广肿瘤微创治疗事业的发展,由上海市抗癌协会肿瘤微创治疗专业委员会、肿瘤绿色治疗新技术联盟及国内著名专家共同主办的第四届肿瘤微创治疗上海论

近年来肿瘤治疗逐渐向微创化、精准化转变。微创治疗技术在肿瘤的诊断、治疗、并发症的处理等方面已发挥不可替代的作用。以肿瘤消融治疗为代表的微创技术以“最小损伤人体结构、有效杀灭肿瘤”为原则,具有创伤小、疗程快、疗效突出等优势,是未来肿瘤治疗的重要方向。

为进一步推动和推广肿瘤微创治疗事业的发展,由上海市抗癌协会肿瘤微创治疗专业委员会、肿瘤绿色治疗新技术联盟及国内著名专家共同主办的第四届肿瘤微创治疗上海论坛暨首届国际消融治疗学术大会和第七届肿瘤绿色治疗新技术论坛于2017年5月5-7日在上海举行。

过去我们认为肿瘤一旦发生转移,就意味着彻底失去根治性治疗的机会,如肠癌发生肝、肺转移,既往以化疗加靶向治疗的全身治疗为唯一治疗方法。而现在经过根治性局部消融治疗的肝脏、肺脏转移灶已经被认为是一种能够治愈的疾病,在其转移消融术后患者可达到根治的效果,这为很大一部分既往无法得到根治的转移性肿瘤患者提供了根治性治疗的机会。而对于肿块较大的转移灶或多发转移灶,可采用肝动脉灌注化疗缩小肿瘤后联合消融治疗,从而使患者获得长期生存。

肿瘤的局部热消融离不开影像引导,而在引导方式的选择上,除了传统的超声、CT引导之外,近年发展起来的基于磁共振自由手三维透视技术引导下的肝肿瘤精准消融提高了肝肿瘤消融治疗的精准性与疗效。

上海市抗癌协会肿瘤微创治疗专业委员会主任委员、复旦大学附属肿瘤医院微创治疗中心主任孟志强教授指出,肿瘤治疗已经逐渐向微创化、精准化转变。微创治疗技术在肿瘤的诊断、治疗、并发症的处理等方面发挥着不可替代的作用。

上述会议内容,除了围绕微创消融治疗这一局部治疗理念之外,也充分探讨了临床如何根据患者疾病治疗所处的不同阶段,采用分阶段治疗,如第一阶段以局部治疗为主,包括局部消融等;第二阶段,全身调理攻补兼施;第三阶段,调整体质治疗的肿瘤绿色治疗理念,更加注重现代微创技术与传统中医药整体治疗的结合。同时,分子影像学是传统医学影像技术与分子生物学等学科相结合的新兴学科,被认为是未来最具有发展潜力的十个医学科学前沿领域之一。

此次大会下设5个分会场,包括消融、海扶、绿色治疗新技术,综合微创治疗,以及护理分会场。在消融分会场,除了传统的肝脏肿瘤消融之外,由复旦大学附属肿瘤医院微创治疗中心的陈颢教授向大家介绍了AI时代的肺消融治疗,包括该中心基于机器人导航的肺肿瘤穿刺及消融,大大提高了穿刺的精准度,减少了反复穿刺可能造成的出血、气胸的并发症。

在海扶分会场,通过与会专家对海扶治疗胰腺癌临床疗效的总结,再次确认了高强度聚焦超声(HIFU)在胰腺癌治疗中的疗效,且因其无创,对于高龄、一般状况差的患者仍可作为优选。

在绿色治疗新技术分会场,局部微创治疗后如何通过中医药治疗进行术后调理与疗效巩固,从而实现绿色、有效治疗。

在综合微创治疗分会场,与会专家对影像技术引导下的I125放射性粒子治疗的适应症、计量控制、疗效判定等方面达成了共识

此次会议的一大亮点是设立了护理分会场,提出针对常见肿瘤微创治疗病例采用个案护理的模式,让微创护理的概念落到实处,包括治疗前的护理、治疗中的护理配合、治疗后的护理、并发症的预防及处理、健康指导、跟踪与随访。
