IBD:磁共振成像I扫描可用于监测TNF抑制剂对克罗恩病的疗效

随着抗肿瘤坏死因子制剂(anti-TNF)的发展,治疗CD的方法有了很大的变化。TNF抑制剂能促进CD患者肠道黏膜的修复,但是这类药物在肠道透壁性炎症及狭窄性病变中的作用基本上是未知的。但是通过内镜检查进行评估其疗效又不太适合,因为其是一项侵入性、耗时、昂贵的检查,特别对于小肠CD.为此来自阿姆斯特丹的Tielbeek等人进行了一项有关MRI评估anti-TNF治疗CD效果的回顾性研究,研究结果发布在8月份的 Inflammatory Bowel Diseases 上。

该研究共纳入50名CD患者(女性54%,中位数年龄37岁),这些患者在接受英夫利昔单抗或者阿达木单抗治疗时进行过MRI检查。基于临床医师的评估、实验室检查及内镜下的表现将研究对象分为治疗有反应组和无反应组。用盲法由两名影像学家应用有效的MRI评分系统对MRI结果进行评分。研究中,在MRI检查中最后共发现64处病灶并对其进行统计学分析,研究数据采用配对T检验和秩和检验。

研究发现,在TNF抑制剂治疗过程中,64个病灶中29个(45.3%)MRI的炎症评分有改善,仍有18个(28.1%)无改变,17个(26.6%)加重。在治疗有反应的组中,MRI中肠道平均炎症程度评分有改善(评分改善:5.19—3.12),狭窄性病变的炎症评分也有显著性改善(6.33-4.58)。但是在治疗无反应组中,平均炎症反应评分并无明显的改变。研究还发现,用MRI来判断TNF抑制剂治疗的反应的准确性为68%.研究认为,接受TNF抑制剂治疗后的CD肠腔中的炎症反应改善程度可以通过MRI扫描进行评估。MRI可以作为评估CD患者透壁性炎症程度的补充性方法,并能够指导平日TNF制剂临床应用。
