JAHA:原子弹幸存者辐射暴露与外周动脉疾病患病率的关系

过去的研究报告表明,0.5至1.0 Gy的全身照射可能是动脉粥样硬化的原因。周围动脉疾病(PAD)是系统性动脉粥样硬化的表现。低至中等剂量辐射的后果是否会导致PAD风险增加仍有待明确。近日,血管疾病领域权威杂志JAHA上发表了一篇研究文章,这项研究的目的是评估辐射暴露与日本原子弹幸存者中PAD患病率之间的关系。

研究人员对3476名原子弹爆炸辐射暴露的参与者(男性占41.1%,平均年龄74.8岁,SD为6.4岁)进行了评估,并在2010年至2014年进行了横断面调查。左侧和右侧踝肱指数和上行时间(UT)通过示波VP-2000仪器检测获得。PAD被定义为1.0或更小的踝肱指数或与血运重建相关的既往病史。UT被认为是早期PAD的敏感标志。研究人员使用多变量回归分析评估辐射暴露与PAD或UT之间的关联,并调整了潜在的混杂因素。

在3476名参与者中,79名(2.3%)参与者被确诊为PAD。多变量logistic回归分析表明,辐射剂量与PAD患病率无关(比值比为0.83; 95%置信区间为[0.57-1.21])。UT似乎随着辐射剂量而增加,但增加没有统计学意义(1.09ms/Gy; 95%置信区间[-0.17至2.36])。

由此可见,该研究发现辐射剂量与PAD之间没有明确的关联,但仍有待确定UT是否与辐射剂量相关。
