J Viral Hepat:Toll样受体4多态性与印度患者感染戊型肝炎病毒的关系

研究首次报道了TLR4多态性与戊型肝炎的关系,并提示HEV感染时TLR4低反应性可能与其多态性有关。

戊型肝炎病毒(HEV)引起的戊型肝炎感染是发展中国家的一项重大公共卫生问题,是成人散发性和流行性急性病毒性肝炎的病因。人们对戊型肝炎感染的发病机理知之甚少。Toll样受体(TLRs)是固有免疫识别病原体相关分子模式(PAMPs)中的关键角色。

在此之前,研究组发现在HEV感染患者中,TLR 4高表达(在蛋白和基因水平上)与LPS刺激单个核细胞后细胞因子反应受损有关。针对TLR4基因(A299G, C399T)多态性与肝病相关性的早期观察,本研究对HEV感染患者的TLR4多态性进行了探究。

研究观察到TLR4-399CC和CT等位基因与戊型肝炎(亚临床和急性患者)显著相关。与对照组相比,TLR4-399 CT在患者类别中的载波频率较低。TLR4-399C等位基因频率较高与疾病进展显著相关。急性肝病患者CG和TA单倍型的频率更高,而罕见的单倍型(TG)在对照组中更高。TLR4-299 (A>G)上的单核苷酸多态性(SNP)没有显示任何差异。

研究首次报道了TLR4多态性与戊型肝炎的关系,并提示HEV感染时TLR4低反应性可能与其多态性有关。
