CSE 2015:宁光教授谈遗传性内分泌疾病的分类和诊断

“对于罕见的内分泌疾病来讲,最需要临床能力,又最具有研究价值,但更重要的是,最缺乏人文关怀,因为疾病给这些患者个人和家庭都带来了很大的损伤。如何给这些患者更多的关怀,确实是我们内分泌医生要做的事情。”宁光教授在8月27日的报告中开门见山地讲道。

这是2015年中华医学会第十四次全国内分泌学学术会议上的一场报告。中华医学会内分泌学分会前任主任委员、上海瑞金医院的宁光教授介绍了罕见内分泌疾病的分类和诊断。

罕见内分泌代谢性疾病具有种类繁多、罹患者众、致死率高、确诊率低的特点。宁光教授将它们分为四大类:低发性疾病、先天性疾病、肿瘤性疾病、遗传性疾病。

Pendred综合症(PDS)是第一个被报告的遗传性内分泌病,此后,更多的遗传内分泌疾病被发现。目前遗传性内分泌病的分类主要是根据临床和解剖,虽然有利于解释疾病的病理生理,但是以分子分型进行疾病分类才能针对该组疾病遗传变异的特征。

随着遗传学的发展,超过90%的遗传性内分泌疾病的致病基因及变异位点已经被发现。结合临床诊断,基因诊断为临床鉴别诊断、发现不典型疾病、以及早期诊断及靶向治疗提供了重要的证据。因而,对于临床疑诊的遗传性内分泌疾病的病例,应该常规应用基因诊断。为了提高遗传性内分泌疾病的精确诊断与治疗,根据致病基因功能,宁光教授提出遗传性内分泌疾病三类十种的分子分类方法:

一、细胞分化:转录因子基因。内分泌腺体起源于特异细胞群体的正常分化,转录因子在时间和空间的严格调控决定细胞分化,如果转录因子出了问题,就会导致腺体分泌异常。

二、激素合成与作用:肽类激素基因、激素合成酶基因、激素合成原料转运基因、膜受体基因、核受体基因、受体后信号传导基因、离子通道基因。

三、肿瘤形成:癌基因、抑癌基因。癌基因的激活与抑癌基因的失活在内分泌肿瘤的形成过程中发挥了重要作用,一个基因突变可致多个腺体受累。

最后,宁光教授强调,要针对遗传性内分泌代谢病特点更全面解决诊断问题,需要5种组学共用:针对激素分泌异常,需要激素组学;针对内分泌腺体异常,需要影像学精准定位;针对激素异常导致的特异症状,需要症状组学,以大数据的方式将疾病症状的组合与疾病相对应;针对基因组数据,需要基因组学;针对表达谱数据,需要转录组学。
