JCEM:高密度脂蛋白胆固醇变异性与糖尿病风险

由此可见,HDL-C的低平均值和高变异性是糖尿病的独立预测因子,具有累加效应。提高和稳定HDL-C可能是降低糖尿病风险的重要靶标。

低HDL-胆固醇(HDL-C)与葡萄糖耐受不良之间的双向关系已得到很好的证实。最近的研究表明脂质变异与各种健康结局有关。近日,内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章,该研究旨在调查HDL-C水平及其变异性对糖尿病风险的综合影响。

该研究为基于人群的队列研究。在2009年至2013年期间,韩国国民健康保险体系队列中共有5114735名未患糖尿病的成年人接受了≥3次健康检查。使用独立于平均值(VIM)和变异系数(CV)的变异性来计算随访间HDL-C的变异程度。低平均值和高变异性组分别被定义为HDL-C平均值和变异性的最低和最高四分位数组。该研究的主要观察指标为新发糖尿病。 

在中位随访5.1年期间,有122192例(2.4%)糖尿病患者。HDL-C较低的平均值或较高的变异性以逐步的方式与较高的糖尿病风险相关,并且注意到两种指标具有累加效应。在多变量调整模型中,高平均值/高VIM组的风险比和95%CI为1.20(1.18-1.22),低平均/低VIM组为1.35(1.33-1.37)以及低平均值/高VIM组为1.40(1.38-1.42),相比于高平均/低VIM组。当使用CV和各种亚组分析对变异性建模时,可观察到类似的结果。 

由此可见,HDL-C的低平均值和高变异性是糖尿病的独立预测因子,具有累加效应。提高和稳定HDL-C可能是降低糖尿病风险的重要靶标。
