Stroke:COVID-19相关缺血性卒中患者凝血和炎症生物标志物

COVID-19相关缺血性卒中可能与COVID-19疾病严重程度和D-二聚体升高的凝血障碍有关。
近日,心血管疾病领域权威杂志Stroke上发表了一篇研究文章,研究人员试图明确炎症和凝血的生物标志物是否有助于将2019年冠状病毒病(COVID-19)相关的缺血性卒中确定为一种急性缺血性卒中(AIS)新的亚型。
研究人员对因严重急性呼吸系统综合症冠状病毒2感染而入院的缺血性卒中患者常见生物标志物进行了机器学习聚类分析,以确定是否与AIS相关。研究人员使用来自3个大型医疗保健系统的汇总数据对结果进行了验证。
基于C反应蛋白、D-二聚体、乳酸脱氢酶、白细胞计数和部分凝血活酶时间的水平,聚类分析将2908名独特的患者并入4种独特的生物标志物表型。最严重的集群表型的患者AIS(3.6%,P<0.001)、院内AIS(53%,P<0.002)、严重AIS(31%,P=0.004)和隐源性AIS(73%,P<0.001)的患病率最高。
D-二聚体是唯一与AIS患病情况独立相关的生物标志物,第4个四分位数与第1个四分位数相比,AIS的风险比高出8倍(P=0.005),这一结果在157名伴有COVID-19的AIS住院患者组成的队列中得到进一步证实。
由此可见,COVID-19相关缺血性卒中可能与COVID-19疾病严重程度和D-二聚体升高的凝血障碍有关。
