Eur Radiol:双相CT检测血栓长度差异能预测急性缺血性卒中的临床预后

目的:血栓长度可能高估了早期动脉计算机断层扫描血管造影(CTA)对侧枝循环状态检测的能力。基于双相CT检测的血栓长度的等级系统,研究人员评估血栓长度的差异与临床预后的关系。方法:招募了M1阻断的急性缺血性卒中患者48名。双相CT检测包括非对照增强CT,具有自动跟踪技术的CTA以及造影剂注射40s后进行延迟对照增强CT扫描。在CTA和CECT中血栓长度差异用三点量表进行分级:G0=没有差异,G1=血

目的:基于血栓长度,目前的医生们可能高估了动脉早期的计算机断层扫描血管造影(CTA)对侧支循环状态检测的能力。基于双相CT检测的血栓长度的等级系统,研究人员评估血栓长度的差异与临床预后的关系。

方法:招募了M1阻断的急性缺血性卒中患者48名。双相CT检测包括非对照增强CT,具有自动跟踪技术的CTA以及在造影剂注射40s后进行的延迟对照增强CT扫描(CECT)。在CTA和CECT中血栓长度差异用三点量表进行分级:G0=没有差异,G1=血栓长度无差异,但是远端血栓信号衰减,G2=血栓长度有差异。进行单变量和多变量分析确定3个月不良临床预后的独立预测因子。

结果:血栓差异等级与侧支状态和DSA前向性血流呈显著线性关系,两者具有较好的一致性。在多变量模型中,血栓长度的差异(G2)是不良临床预后的独立预测因子(OR=11.474 (1.350-97.547))。

结论:双相CT检测血栓长度的差异可能是在急性M1阻断患者不良临床预后方面的有用预测因子。
