Lancet:肾移植供体供氧保存对移植患者预后及肾功能的影响

供体肾源采用低温机器灌注补充氧气保存是安全的,虽然与常规保存方式相比,12个月时患者肾小球滤过率差异不显著,但可减少移植后并发症和移植失败风险

用于移植的肾脏供体的保存一般采用寒冷,低氧条件下。在保存过程中提供氧气可能会减少肾脏损伤,改善移植后的结果。近日研究人员考察了低温机器灌注(HMP)期间补充氧气对肾脏移植患者预后的影响。
 
本次随机,双盲,匹配的III期临床研究在欧洲19个移植中心开展,肾脏供体来自50岁或50岁以上的捐赠者,捐赠者循环死亡后,两个肾脏被移植到两个不同的接受者中,其中一个肾脏进行HMP供氧保存(HMPO2 组),另一个进行无氧保存(HMP组)。研究的主要终点为12个月的受体患者肾小球滤过率(EGFR)差异。
 
106对患者参与研究。12个月时,HMPO2 组的平均EGFR为50.5 mL/min /1.73m2,而HMP组为46.7 mL/min /1.73m2。HMPO2 组严重并发症事件较少(Clavien-Dindo IIIb及以上,11% vs 16%),移植失败率较低(3% vs 10%,HR=0.27)。
 
研究认为,供体肾源采用低温机器灌注补充氧气保存是安全的,虽然与常规保存方式相比,12个月时患者肾小球滤过率差异不显著,但可减少移植后并发症和移植失败风险。
